During the opening credits, Matool runs through the deserted streets of Albuquerque as Nathan drives his pickup, emphasizing that civilization has collapsed. The scenes establish Matool's fearless nature and the environmental emptiness of the city.

A young woman is trapped in her car while zombies close in and her companion is devoured. Matool rushes to her aid, shouting in his typical profanity, and carries her to safety. They seek refuge in a rundown, abandoned house.

Inside the house, Rainville and two boys are present. Dexy explains that the killed man is her brother, triggering awkward attempts at intimacy from Matool. As Dexy attempts to leave to save her brother, she is dragged down by the zombies while Rainville and one boy are eaten; the quiet boy hides.

Nathan hits Matool with the truck and drives him to his mountain home, where Matool wakes bound in the back along with the quiet boy. Inside, Matool is freed, given a bath, and learns that Nathan and Dexy practice an alternative lifestyle in which Nathan watches while other men have sex with Dexy.

Matool spends time with Dexy's sister Sassy, who spanks him with a ruler for the first time. He then discovers Dottie, a mutated conjoined twin face on Sassy’s side, which unsettles him. The interaction foreshadows the bizarre undercurrents in Dexy's world.

Behind the house, Nathan keeps a naked female zombie in a shed, which he looks at with strange fascination. Matool remarks that she would be hot if she weren't decomposing, highlighting the group's chilling appetites. The scene hints at the moral decay running through the household.

A rain-soaked entry reveals Mandel, Vega, and a badly injured Sepulveda who join the household. A flashback reveals their unit's collapse on the road after hyperzombie attacks, explaining the survivors' paranoia. Mandel learns that the house is well-supplied and defended by a generator and a well.

Mandel and Vega begin having sex with Dexy while Nathan watches, provoking his jealousy. In response, Nathan returns to the shed and has rough sex with his chained zombie partner, a backstory that underlines the group's twisted sexuality and survival tactics.

Vega forms a relationship with Sassy as Sepulveda's torment continues; Mandel resists euthanizing him, showing the fragility of morality in a world overrun by zombies. Dexy's attraction to the strange household grows, adding to the mounting tension.

As zombies crowd the house, Mandel tries to reclaim the MP5 but cannot reach it as the boy locks them out. They find Nathan devoured in the shed, and his zombie is contained again. Dexy reassures herself with the line 'Nathan will be back' as the group runs down a gravel road to escape with the boy holding Dexy’s hand.
