George Grieves [Tom Cavanagh] checks into the Mt. Abaddon Hospital for what seems like a routine procedure, only to step into a corridor of whispered dread and unsettling uncertainty. He awakens after what should have been a simple colonoscopy, and the medical staff reveal a chaotic mix-up rooted in similar patient names. Instead of the expected exam, he was mistakenly given a sympathectomy intended to cure sweaty palms, a mistake that twists his world into something unthinkably wrong.

As days bleed into nights, Grieves’ post-operative experiences grow stranger and more unnerving, each moment a deeper dive into a nightmare born from his own mind. He comes to realize that he is trapped inside a convulsing netherworld of his worst fears, a limbo he cannot escape or wake from. The boundaries between patient and nightmare blur, and he begins to understand that something in his recovery has gone catastrophically awry, sealing him in a psychic prison rather than letting him return to the living world.

From the outside, the picture grows bleaker. Doctors explain a grave medical outcome: a complication from the colonoscopy has released an air bubble into his bloodstream, which traveled to his brain and left him in a seemingly permanent vegetative state. He has already been gone from the world for months—ten months, in fact—and his family faces mounting pressure to disconnect life-sustaining support. The courtroom of consensus around his bedside grows louder, while inside his mind, the battle continues in a more intimate, terrifying form.

Inside Grieves’ inner landscape, the struggle against his own fears intensifies. He fights to understand the rules of this dreamlike reality and searches for a way to escape the torment. The only apparent exit seems to be slipping away from the waking world through a final act—suicide within the dream—to force an end to the ceaseless torment and, perhaps, to end the life of the body that remains unresponsive in the real world. He makes a desperate leap from a seventh-floor window, crashing into concrete below, while the last image on screen is his body lying still in an empty hospital room, its eyelids closing as life finally ends outside the dream.
