Paranormal investigator group known as the Paranormal Paranoids—Riley, Laura, David, and Peter—vanish during a probe at Shelby Oaks prison. Only Riley remains unaccounted for, while Laura, David, and Peter's bodies are later found. A recovered mini-DV camera shows a terrified Riley, hinting that something unseen took the others before Riley disappeared.

Seventeen years after the disappearance, Mia is interviewed for a documentary about Riley. She insists that Riley could still be alive and recounts Riley's childhood night terrors, when something seemed to be watching her. The interview strains Mia's marriage and her hopes of conceiving a child, underscoring the unresolved case.

A stranger arrives at Mia's door, murmuring 'she finally let me go' before shooting himself. The unsettling encounter signals that someone connected to Riley's case has resurfaced and left a chilling message behind.

Mia retrieves a mini-DV tape labeled Shelby Oaks from the stranger's body. The footage shows the Paranoids investigating the prison and an abandoned amusement park, then being killed or overpowered by a masked attacker who captures Riley; the recording cuts out before the fate of Riley is shown.

That night, Mia glimpses a dog with glowing eyes prowling outside her house. The eerie sight reinforces the sense that the Shelby Oaks case is far from over and that something supernatural is in play.

Mia interviews the former prison warden, who identifies the killer as Wilson Miles, a violent offender. He explains that Miles arrived during a riot, and a computer malfunction opened every cell, yet Miles stood at his window and did not leave as the chaos unfolded.

Mia finds a book in Riley's belongings describing demonic symbols. After researching parasitic incubi and hellhounds, she becomes convinced Riley was the victim of an occult ritual rather than a mere disappearance. Robert, overwhelmed by Mia's fixation, ultimately leaves.

Mia travels to Shelby Oaks, finds Miles' former cell, and looks out toward the ruins of the amusement park. She is attacked by a hellhound, escapes to the amusement park, and encounters a dilapidated farmhouse where she meets the elderly Norma.

In Norma's house, Mia discovers that Norma is Miles's mother and that Miles kidnapped Riley years earlier. The implication is that a long-running, occult-driven plot kept Riley in captivity for Tarion's purposes.

Under Norma's influence, Miles—Tarion's devotee—repeatedly rapes Riley to conceive a child. After many miscarriages, Riley finally bears a healthy baby boy, revealing the horrifying ritual at the heart of her past.

Mia texts Robert to send police to her location, hoping to rescue Riley and bring an end to the nightmare. The mounting danger underscores the urgency of confronting the occult threat.

Norma lures Mia into the basement where Riley is imprisoned. Mia frees her, but Riley grabs a fire poker and confronts Norma upstairs. Norma incapacitates Riley and completes a blood ritual over the newborn, with Tarion briefly glimpsed before Norma dies from blood loss.

Riley and the baby are taken to the hospital and discharged that night. The healing process is fragile, and the baby’s safety remains precarious in the wake of Riley's trauma and Norma's death.

That night, Riley attempts to smother the baby, insisting the child is possessed. Mia intervenes, and in the struggle, she accidentally pushes Riley out the window. Riley survives the fall but is surrounded and devoured by hellhounds as Tarion appears behind Mia; Mia's eyes glow and she screams, signaling Tarion's ongoing influence and a new vessel is poised to emerge.
