A quiet, disturbing tragedy unfolds early on as a young boy named Matthew accidentally turns his father’s tractor on while the man works the land, an act that proves fatal and leaves Matthew with a mangled left hand that is replaced by a brutal hook. This life-altering accident sets a grim tone for what follows: Matthew is placed in a psychiatric hospital where the injury and his growing detachment become central to his identity. Upon release at eighteen, he returns home to a world that has shifted in his absence. His mother has married their neighbor, Mack Parsons, and the family dynamics are forever altered. The youthful boy who once played beside the fields now harbors unsettling, almost forbidden feelings toward his mother, blending discomfort with an aversion to sex that clouds his impulses. When he confronts Parsons, Matthew’s resentment erupts into violence, and he murders Parsons with an axe. The encounter that follows is equally brutal: a heated argument with his mother ends with her dying after she is knocked to the ground and hits her head on a rock.

The following morning, Matthew’s course grows even darker as he hitchhikes with a young couple, whom he also kills—beating the man’s head with a rock and drowning the woman in a roadside stream. His violence becomes a disturbing pattern that leads him to a town where he encounters a painter and prostitute named Vera, a figure who seems to echo the maternal image he cannot reconcile with. This meeting deepens his fixation and drives him toward ever more extreme acts. He murders a drunken sailor who visits Vera’s circle in a fit of jealousy, an act that foreshadows the way his obsession will warp his sense of control and protection. In an attempt to impress Vera and provide her with a life that he believes she deserves, Matthew fraudulently gains entry to a grand mansion. There, he murders the owner with a pillow, slashes a maid with a cleaver, and even beheads the resident’s pet dog. The brutality escalates, and the couple’s fragile dynamic becomes a stage for Matthew’s escalating delusions.

With Vera now at the center of his warped world, Matthew brings her to the mansion and imagines a future where she will share his life. When Vera resists his visions and refuses to live with him, his temper flares, and he imprisons her. He attempts to force a domestic life upon them, but Vera remains defiant, even after he bludgeons a doctor who visits the house. Vera’s awareness of Matthew’s deep-seated disgust with sexuality becomes a strategic weapon; she uses his aversion to manipulate him, pressing him to act out his desires. In a tense turn, she provokes him to attempt sex, and while his attention is diverted, she stabs him with a loose door hook in a bid to escape. Matthew catches her and, in a final, brutal moment, rips her throat out with his hook.
