The film opens in a stark and desolate ghetto. A half-naked man is seen struggling as he types away at an old typewriter, setting the tone for a grim reality in which characters navigate life and death.

In 2025, the corporate entity known as The Union has perfected the creation of bio-mechanical organs, but they are primarily accessible to the wealthy and those with excellent credit scores. This creates a growing divide where those without resources risk severe consequences for their medical needs.

Customers who default on payments for their artificial organs face dire repercussions, including the dispatch of a Repo man to reclaim the organ. This process not only affects financial status but often results in the customer's death, underscoring the ruthless nature of the corporate system.

Among the elite Repo men are Remy and his partner Jake Freivald, who embrace their grueling jobs despite the emotional toll it takes on their lives. Their work serves as a constant reminder of the ethical dilemmas they face, driving a wedge between Remy and his wife, Carol.

After a harrowing encounter at a Nest, Remy and Jake receive a promotion to full-time raid captains. Their new title comes with increased responsibilities, further intertwining them with the corporate ethos of The Union.

In a pivotal moment, Remy is tasked with repossessing the heart of musician T-Bone, someone he personally admires. This assignment hits close to home, representing the loss and tragedy inherent in his job among the wealth and privilege of his clientele.

A tragic malfunction with a faulty defibrillator results in Remy suffering severe injuries. This incident forces him to receive an artificial heart, drastically altering his perspective on his work as he starts to empathize with the victims of his repossessions.

Following his painful divorce from Carol, Remy finds himself temporarily living with Jake. As he grapples with the fallout from his personal life, he struggles to succeed in a sales role, realizing he cannot mislead clients about the grave consequences of defaulting.

Faced with insurmountable debts, Remy reluctantly returns to Repo work. His new artificial heart deepens his emotional conflict, making it increasingly difficult for him to execute his orders without hesitation.

At the Pink Door, Remy and Beth attempt to erase their debts through a painful act of self-sacrifice. This critical moment raises questions about morality, consequence, and the nature of human experience in their fight for freedom.
