The film begins with a captivating sequence of comic-book illustrations that reveal a devastating global epidemic of organ failures in a bleak future. Out of this chaos arises GeneCo, a billion-dollar biotechnology company that profits from organ transplantation. With financial options for their customers, GeneCo also holds the power to repossess organs from those who fall behind on payments. This is where the “organ repo men,” lethal enforcers working for GeneCo, come into play, relentlessly reclaiming the company’s assets by any means necessary. One such Repo Man ruthlessly stalks and eliminates a client, claiming their heart for GeneCo.
Meanwhile, Rotti Largo, the company’s president portrayed by Paul Sorvino, discovers he is terminally ill. In a separate struggle, Shilo Wallace, a 17-year-old girl suffering from a rare blood disease, sneaks through hidden underground tunnels to visit her mother’s mausoleum, wondering about her mysterious heritage. In a moment of curiosity, she follows a bug out of the mausoleum, leading her to the GraveRobber—a character engaged in sinister dealings of his own. As they evade the GenCops, they enter a vast underground graveyard, setting the stage for Shilo’s journey.
After collapsing from blood pressure issues, Shilo awakens in the presence of her overprotective father, Nathan Wallace, brilliantly played by Anthony Head, who has sheltered her for 17 years due to her illness. Amidst family drama, Rotti’s children, Luigi, Pavi, and Amber Sweet, gather at a GeneCo facility, squabbling over who will inherit the company upon their father’s death. Rotti takes a keen interest in Shilo, inviting her to attend the Genetic Opera, while Nathan is on a dangerous assignment to reclaim a defaulted organ, seemingly unaware of the escalating tension.
As the plot unfolds, Amber Sweet, desperate to maintain her beauty, becomes embroiled in the world of Zydrate—a powerful and addictive painkiller. The evening of the Genetic Opera arrives, where everything culminates in a deadly performance. As Blind Mag, a former star with a profound connection to Shilo, sings her final song on stage, shocking revelations unfold about lineage and the dark secrets that bond them.
In a tragic confrontation, Shilo learns the truth about her father’s past actions and the sinister intentions of Rotti.
