In Madrid, 2011, Professor Fuentes binds five medical students to chairs and explains he will test a power beyond conventional medicine. He selects Jose Aguas as a volunteer due to his cockiness and sets up the demonstration. An emergency cabinet with pig blood and a penknife is revealed, and Fuentes asserts Jose must stop the bleeding or die, proving the mind’s control over the body.

Fuentes cuts Jose, but no real bleeding occurs: the bag of pig blood leaks subtly instead. He demonstrates that the mind can suppress or induce symptoms at will. Fuentes then leaves, reminding the students that the next week’s exams will require them to stay calm under pressure.

Ángela watches Fuentes acknowledge the lesson but warn that her grant may fail if she doesn’t perform. Fuentes hints the experiment could help her future, and Ángela spots newspaper clippings about the ghost town Susurro, sparking a plan to study it scientifically.

Ángela proposes a controlled study of Susurro to prove fear is mentally driven. She ropes in Belén and Toni, uses Carlos to help with equipment, and reminds herself of the personal stakes as the sisters’ fragile bond begins to surface through shared memories.

To transport gear, Ángela asks her sister Diana to lend her a van. A flashback shows the sisters as girls with a distant father, foreshadowing the trauma that will influence their decisions. Diana reluctantly agrees, while the group’s dynamics deepen with the looming danger.

The crew travels to Susurro with equipment in tow. They stop at an oil station where Toni becomes suspicious of others’ motives, and Belén voices practical concerns. They decide to proceed to the salt mines, hoping to connect Matarga’s legend with the town’s hauntings.

The group reaches Susurro, a town of empty buildings and strong atmosphere. They choose to spend the night in the old church at the town center, hoping its structure will shield them from malevolent forces. The teams pair off to explore, establishing the first real tension between them.

Diana volunteers for a hypnotic session using symbolic prompts while Toni directs. Through the mist, Dr. Matarga materializes with a single eye, questioning whether Diana truly wants to remember the past. His appearance triggers a chilling memory and a sense that something locked away is about to break free.

The group tracks down Matarga’s former residence, where Carlos is seized and subjected to a brutal torture setup. Diana’s memories of their father’s brutality surface, intertwining with Ángela’s resolve and signaling a turning point in how the sisters confront their past.

Believing there is a way to reach help, the group discovers a secret passage linking the torture chamber to the salt mines. They attempt to move Carlos toward safety, but a flat tire in the van stalls their escape, heightening the peril as Matarga's influence grows stronger.

While the group struggles with the van, Belén is dragged from the vehicle by Matarga and killed, her body left behind as the others scramble to regroup. The loss intensifies the group’s fear and desperation, forcing them to face the horror inside Susurro.

Back in the church, Jose is found hanging from a hook after trying to console Diana; his face is bound with wire. The surviving students review the footage and realize Diana may be in some form of possession or merging with Matarga, deepening the mystery and fear among them.

Diana returns from the memory-driven state and, under Matarga’s influence, confronts Toni in the church. She slashes his fingers and then his neck, revealing that the haunting has become a personal and violent reality for the group.

Angela arrives to intervene as Diana remembers how their father treated them and the truth about their family tragedies surfaces. Matarga stabs Ángela, and in a final act of defiance, Diana cuts her own throat to stop him, killing Matarga in the process. Angela collapses, wounded, and Diana dies in her sister’s embrace.

Angela staggers toward an oil station, exhausted and alone, where she encounters the aftermath of the night’s horrors. Fuentes later meets her at the garden of a psychiatric institution and hands her an envelope holding her father’s watch, hinting at a continuing burden. A doctor’s voice, speaking through Matarga, implies that remembering what happened will haunt Ángela for life.
