Mathew Harris, Paul McGann, is a porter in a busy London hospital who secretly dreams of bigger opportunities. He spends his evenings strumming a banjo in his cramped bedroom, still smarting from a choice he once made to drop out of biology studies. One fateful night, he witnesses a car crash outside a local pub—the driver is killed in a collision with a lorry. Harris knows the dead man, Simon Hennessey, a doctor at his hospital, and soon realizes Hennessey had been pursuing a residency at another hospital in Bristol. Driven by ambition and a hunger for a fresh start, Harris seizes the moment: he takes the deceased man’s mail, crafts a job application, and applies for the position under the late doctor’s identity. After poring over medical texts and quietly observing procedures at his own hospital, Harris is accepted for the new role.

Before departing, he seeks out his friend Alec Moran, Jimmy Yuill, at the pool and explains he’s traveling abroad. To keep the illusion, he hands Moran his silver cigarette case and quietly quits his job, heading to Bristol where he will take on the alias Dr. Simon Hennessey, Dale Rapley, in a bustling Accident & Emergency (A&E) department.

In Bristol, Harris’s inexperience is tempered by a mounting confidence that comes with authority. He faces his first real tests as a physician and is quickly reprimanded by a supervisor for sloppy patient record-keeping, a fault that threatens to derail his bold new life. Amid the stress, he encounters a compassionate nurse, Christine Taylor, Amanda Donohoe, whose warmth and keen perception draw him in. She becomes a stabilizing force for him, offering guidance through the chaos of the A&E and, increasingly, a personal connection that blossoms into a complicated affair. Harris savors the winning streak of saving a life and the respect that comes with his apparent competence, yet his arrogance grows with the power he wields.

The romance complicates matters when a patient—the wife of the chief medic—dies while Harris is in charge. The tragedy triggers a formal charge of negligence, and the hospital’s scrutiny intensifies. At the hearing, Christine Taylor steps forward to shoulder the blame, a selfless act that forces her to resign from her post. The affair has left consequences, and the rift between professional duty and personal desire widens.

A turning point arrives when Harris unexpectedly encounters Moran, who has relocated from London to pursue nursing studies. Harris lures Moran to a remote area and confesses the truth of his impersonation, begging Moran to keep the secret and leave town. Moran’s fear of exposure turns to defiance, and he threatens to report Harris. The confrontation escalates tragically when Harris follows Moran and pushes him off a cliff. Moran survives the fall but is gravely injured, and he is rushed back to the hospital for treatment. The investigation intensifies as a nurse discovers a silver cigarette case engraved with Harris’s name, suggesting the impostor’s identity to be at risk of exposure.

In a desperate bid to erase the threat Moran poses, Harris attempts a second, deadly act in the A&E: he injects a blood bag intended for Moran with 20 percent potassium chloride, a lethal dose that seals Moran’s fate in the operating room. The incident sends shockwaves through the department and deepens the moral crisis Harris faces.

Christine Taylor, increasingly convinced of Harris’s disguise, pieces together the truth when she visits London and spies the gravestone of the real Dr. Simon Hennessey. Confronting Harris with the evidence, she forces a reckoning with his deception. Yet Harris uses his charm and manipulation to twist her into identifying Moran as Harris, an act that would allow him to be legally declared dead and continue living under the doctor’s identity.

As the hospital’s scrutiny grows, Harris transfers to another hospital in nearby Salisbury, hoping to escape the fallout. He agrees with Taylor that the impersonation must end and that they should move toward a life in London together. Yet in a final, stubborn move, Harris heads toward Salisbury instead of following through with the plan to vanish into a new life with Taylor. In the closing moments, Taylor hesitates—taunted by the possibility that reporting Harris would also expose her own misidentification of Moran. Meanwhile, Harris persists in his impersonation at the new Salisbury hospital, leaving his future and theirs hanging in the balance.

In this tense, morally charged drama, the boundary between competence and deceit blurs as ambition, love, and a dangerous hunger for power push Harris toward an increasingly perilous edge. The film intertwines medical peril with ethical collapse, painting a stark portrait of a man who dares to wear another life like a badge while the people closest to him pay a heavy price.
