An older man calling himself Okka Magadu (also Balakrishna) claims responsibility, appearing as Satyam’s future self. Their enemy is politician Namboodriyar (Ashutosh Rana), whose son elopes with a middle‑class girl under Satyam’s protection. The plot mirrors the 1996 film Bharateeyudu, and the movie was a blockbuster hit.

Set in a remote, sacred borderland, Veera Venkata Satyanarayana Swamy Ashutosh Rana presides as an unyielding guardian of the region, a sanctuary locals treat as a living deity. He shares a quiet life with his grandmother and treats the spirited Sarada Simran as if she were still a child, a dynamic that hints at deeper loyalties and hidden ties. When Satyanarayana steps in to defend Chief Minister Namboodriyar Kota Srinivasa Rao’s son, who faces opposition to his relationship with his girlfriend, he takes a bold stand. After rescuing him from the CM’s goons, Satyanarayana confronts the Chief Minister, whose fury swells at the sight of Satyanarayana’s vast cadre and unwavering resolve. Meanwhile, Bhavani Anushka Shetty, the CM’s daughter, looks at Satyanarayana with desire, while Aliveelu Priyanka Kothari, a journalist, also vies for his attention, setting up a clash of loyalties and ambitions in the town.
Against this backdrop, a new threat looms: a 101-year-old legend named Okka Magadu Nandamuri Balakrishna begins to strike, stirring public fear as he vanishes and supposedly returns to kill criminals whose bodies vanish. The government assigns CBI officer AK Ravi Prakash to unravel the mystery, and a trap is set to capture the supposed invincible killer. A CCTV clip of Okka Magadu abducting a judge triggers the investigation, and AK concludes that the man cannot be the elderly figure they know; the body language reveals a younger, more martial energy. The search narrows to a village where Satyanarayana’s name suddenly emerges as the man behind the mask.
When the police close in, Okka Mogadu, Satyanarayana’s own identity as the masked warrior, arrives and clears Satyanarayana of homicide charges, revealing himself as a patriot rather than a villain. He escapes with a warning about a traitor in their midst. The tale then reveals Satyanarayana’s ultimate family lineage: the true Okka Magadu is his grandfather, a man who dedicated his life to the country. The villagers learn that Okka Magadu is deeply committed to justice and to Sarada, who was pregnant when he departed decades earlier.
Okka Magadu is portrayed as a benevolent doctor in the pre-independence era, treating people equally and upholding the sanctity of human life. He stands against the British authority led by the cruel Douglas, who orders brutal acts against rebels. Okka Magadu defeats Douglas and becomes a symbol of resistance, protecting Sarada and joining the revolutionary movement that ultimately leads to independence. When the revolutionaries surrender as prisoners of war, Okka Magadu promises to return to the nation and to Sarada, who awaits him as a mother-to-be.
Sixty years later, Okka Magadu escapes from prison and finds a country that has changed. He confronts the reality of the modern era and is pleased to discover that Satyanarayana is proud to be descended from such a man. He retrieves ill-gotten gains looted from a Swiss bank by the CM and donates them to charitable causes, turning public opinion against the corrupt leadership. The Chief Minister uses a political protest to mask the truth, triggering a surge of suicides as the people fall into poverty. Okka Magadu then appears in a public stadium, exposing the CM and presenting the outlaws who were to be slain. The criminals attempt to flee by air, but Okka Magadu brings them down, ending their flight in a fiery crash. A reporter later finds Sarada and declares that Okka Magadu will return.
The final image follows Okka Magadu walking between mountains beside the ruins of a crashed plane, a silhouette of a hero who defied time and tyranny.
