Satya Balram Azad, portrayed by John Abraham, holds the position of home minister in a coalition government in Lucknow, Uttar Pradesh. Facing opposition from Yashwant Paswan, the head of the opposition, Satya attempts to pass an anti-corruption bill, but he finds himself at odds with his own allies. To complicate matters further, his wife, Vidya Azad, the daughter of Chief Minister Chandra Prakash, also votes against the bill.
For 25 long years, Satya’s mother, Suhasini Devi Azad, has remained in a coma, presenting an emotional backdrop to Satya’s political struggles. As tensions rise, a crisis erupts when doctors at a government hospital strike, refusing to treat an injured girl. This results in the girl’s tragic death right before the eyes of protesting doctors and the media, igniting outrage. In a dramatic turn of events, a vigilante emerges, eliminating the strike’s leader — a doctor implicated in a scheme to boost business for his private hospital. To the shock of many, this avenger is revealed to be Satya himself.
Unmoved by the escalating chaos, DCP Upadhyaya is tasked by Chandra Prakash to investigate the vigilante’s identity. The case is handed over to Satya’s twin brother, Jay Balram Azad, who is known for his unpredictable temper and disregard for the law. Accompanying Jay is his assistant, Gyaneshwar Chaurasia. Meanwhile, a scandal unfolds as children fall gravely ill after consuming tainted food at a madrassa, leading to a shortage of oxygen in the hospital where many are treated, causing the deaths of 40 innocent children.
Determined to unveil corruption, Satya discovers that the spoiled food was supplied by a relation of Parag Tripathi and that a key associate of Shankar Prasad was responsible for the oxygen shortage. Both men represent the same alliance party and threaten to withdraw their support from Chandra’s government when pressed by Satya. In a shocking act of retribution, Satya executes both Tripathi and Shankar by using the very food that harmed the children.
Then, another disaster strikes as a flyover collapses, and the contractor, Madan Lal Joshi, attempts to evade accountability. Satya’s plan to abduct Madan is thwarted when Jay tracks them to a remote warehouse. In a tense confrontation, the brothers unite against their common enemy and eliminate Madan together.
Their connection to their late father, Dadasaheb Balram Azad, surfaces as they recall his legacy of fighting for the rights of marginalized communities. Dadasaheb’s tragic demise, marked by his relentless battle against Yashwant Paswan’s corruption, fuels their resolve to act. In a series of violent retributions for social justice, Vidya witnesses rampant injustices, including the tragic self-immolation of a woman wronged by local politicians.
With Suhasini awakening from her coma and revealing that Chandra Prakash, not Yashwant, was responsible for Dadasaheb’s death, the stakes heighten. In a climactic showdown at a Yagya in Varanasi, Jay and Satya are forced to confront their father’s killer. Chandra’s attempt to escape with Suhasini’s life at stake leads to a brutal clash between the brothers. However, their mother’s unwavering spirit inspires a revolt against Chandra, culminating in his capture and execution by an enraged mob. The saga of revenge, sacrifice, and relentless courage embodies the essence of justice as the brothers reclaim their family’s honor.
