The film opens with a series of radio transmissions and police chatter, revealing a cave-in at the north side of the Hanniger Mining Co. Rescue teams discover six miners trapped in the shaft—five are dead, and one, Harry Warden (portrayed by Richard John Walters), lies comatose. The local media begins to speculate that Harry went insane and killed the other miners to survive longer, citing the gruesome state of some of the bodies as evidence.
One year later, on Valentine’s Day, we revisit Harry’s final moments before the collapse, where he warns Tom (Jensen Ackles), the boss’s son, not to make mistakes. As fate would have it, a catastrophic event occurs, leaving Tom as one of the few survivors. Suddenly, Harry wakes up, confused and disoriented, and in a shocking twist, a nurse who enters his room meets a grisly fate at his hands.
Soon we see Sheriff Burke (played by Tom Atkins) and a deputy investigating the carnage at the hospital, where they discover a nurse’s heart left on a counter with the haunting message, > “HAPPY VALENTINE’S DAY.” The sheriff deduces that Harry is on the loose and likely headed back to the mine, where a wild student party is underway. Among the high schoolers, Axel (Kerr Smith) is making out with Irene (Betsy Rue), while Sarah (Jaime King) arrives with Tom, adding tension between the two boys. As the group splits up in typical horror movie fashion, Sarah is soon separated and winds up in a deadly encounter with Harry, resulting in chaos as Tom and the others desperately try to escape his wrath.
Fast forward ten years, and Axel, now sheriff, recalls the chilling anniversary of the Valentine’s Day Massacre while dining at a diner. His wife, Sarah, still haunted by the past, is now working at a local grocery store. Axel’s life takes a torrid turn when it’s revealed he is cheating on Sarah with her coworker, Megan (Megan Boone). Meanwhile, Tom is back in town, attempting to sell the mine and escape his memories, but his return dredges up remnants of their shared tragedy.
Things spiral downward when a series of brutal murders, echoing the past, sets the town on edge. Bodies start piling up, and it becomes apparent that Harry’s spirit—or perhaps something more sinister—has resurrected the horror. Axel, desperate to protect his family and uncover the truth, embarks on a dangerous investigation which leads to heart-wrenching secrets that could destroy them all.
As the film unfolds, secrets spill out: Axel’s infidelity with Megan, Tom’s mental health struggles, and the long-buried truth about Harry Warden. The lines blur between past and present horrors as Sarah grapples with the realization that everything she holds dear is at stake. As they navigate through twisted friendships, betrayal, and shocking confrontations, they must ultimately confront their demons to survive.
