As John Hatcher (Steven Seagal) and his partner undertake an unyielding pursuit of the notorious Hector, their journey takes a perilous turn. The tension escalates when Hector’s treachery is unveiled, leading to the tragic murder of John’s partner during a botched drug deal. Overwhelmed by grief, John decides to hang up his DEA badge for good and seeks solace in the familiarity of his home.

Yet, the shadows of his past haunt him, compelling John to re-enter a troubling situation when he visits an old friend, Max Keller (Tom Wright). Upon arrival, he discovers a crew of threatening Jamaican drug dealers, known as the Posse, hovering ominously near the school where Max teaches. The situation intensifies as they encounter each other at a local bar, culminating in a violent shootout that forces John to step in to help capture one of the criminals.

As law enforcement arrives at the scene, an FBI agent approaches John with a tempting offer to join the fight against the Jamaican cartel. However, burdened by his recent loss, John refuses their assistance. Meanwhile, a vigilant detective uncovers a haunting symbol at the crime site—a religious icon that serves as a grim signature left by the infamous Screwface.

As John digs deeper into the enigma, he becomes the target of Screwface’s minions, experiencing a harrowing drive-by assault on his sister’s residence, which leaves his niece Tracey (actress) seriously injured. With his family now in jeopardy, John confronts Jimmy Fingers, a dubious figure connected to the nightclub incident, but the gangster stays mum, leaving John no choice but to take extreme actions.

An enigmatic hint from one of Screwface’s followers only thickens the plot, pulling John further into a labyrinth of betrayal and danger. His sister’s home faces another looming attack, with the atrocious threat of a ritualistic sacrifice just moments away. John arrives in the nick of time to prevent the horrific act, but Screwface manages to slip away, escaping into the shadows.

Desperate for answers, John recruits Max to aid him in tracking down Screwface’s gang, determined to unveil the truth behind the ominous symbols trailing him. An exhilarating chase erupts through an upscale shopping district, where John and Max clash with several of Screwface’s henchmen in a brutal confrontation. Although they fail to extract any significant information from the confrontations, John’s resolve remains steadfast.

The battle escalates as Screwface seeks revenge on John, cornering him between two construction vehicles and igniting his car with a Molotov cocktail. Yet, John’s tenacity allows him to escape the fiery chaos, forever altered by the experiences he endures.

In a critical moment, Charles Marks (a character portrayed by an actor) - a seasoned Jamaican detective collaborating with the FBI to dismantle Screwface’s operation - extends a hand to John and Max. Together, they embark on a treacherous mission to Jamaica, where word has it that Screwface has taken refuge. With the aid of a former lover connected to Charles’ past, they obtain crucial intelligence that reveals a chilling fact: Screwface possesses not one, but two heads and four eyes—a disturbing revelation that sharpens their focus on his Jamaican hideout.

Under the veil of night, the trio instigates a daring raid on Screwface’s heavily fortified complex amidst a masquerade party. John employs an explosive device to distract the guards and disables the power supply. This allows Max and Charles to unleash a storm of gunfire upon the Posse as they storm the sanctuary. During his infiltration of the lavish gathering, John is captured, preparing for a gruesome end at the hands of Screwface. However, he resists impending doom, breaking free from his constraints and engaging Screwface in a fierce sword duel that concludes with the villain’s decapitation.

With their primary goal reached, John, Max, and the remnants of Charles’ team return to Chicago, parading Screwface’s severed head to intimidate the remaining gang members into submission. Just as it appears they have gained the upper hand, a shocking twist unfolds—Screwface reemerges, igniting a brutal gunfight that claims Charles’ life. In the aftermath of the chaos, Max provides cover for John’s relentless pursuit of Screwface, who is ultimately revealed to be the twin brother accountable for the Posse’s vicious crimes across the country.

After a grueling confrontation, John gains the advantage, blinding and sending Screwface plummeting down an elevator shaft, where his lifeless body is gruesomely impaled—a chilling reminder of the depths of his villainy. As John and Max leave the scene, carrying the weight of Charles’ death, they are forced to confront the grim reality of their quest for justice and the profound human cost it incurred.
