In 1653 Salem, Massachusetts, a determined Reverend Traske banishes the headstrong Winifred Sanderson - portrayed by Bette Midler - after she boldly rejects the idea of marrying John Pritchett. Refusing to be separated from her beloved sisters, Mary and Sarah, she leads them to a secluded forest. There, they encounter Mother Witch, who bestows upon Winifred a mystical book bound in human skin, featuring a living eyeball as its centerpiece. However, Mother Witch warns the sisters against the powerful Magicae Maxima spell, which grants absolute power, while teaching them the dark arts of maintaining their youth through the exploitation of children. Seeking vengeance, the sisters unleash their wrath on the Reverend, cursing his home until it is consumed by flames.

Fast forward to 2022, nearly three decades after the tragic return of the Sanderson sisters, the local teenagers Becca and Izzy find themselves preparing for Becca’s sweet sixteen amidst Halloween festivities. They decline a party invitation from their estranged friend, Cassie Traske, and instead explore a magic shop located in the Sanderson cottage. There, they meet Gilbert - played by Sam Richardson - who gifts Becca a candle that turns out to be another Black Flame Candle. With a full moon overhead and their virginity intact, the candle inadvertently resurrects the Sanderson sisters once more.

As the sisters, now back in the modern world, pursue the girls through a local Walgreens, the girls manage to outsmart them but quickly realize that they have been tricked by Gilbert, who was inspired by their 1993 Halloween exploits and misled them into reviving the witches using information from the spellbook.

Upon spotting a flyer for Mayor Jefry Traske - Cassie’s father and a direct descendant of the Reverend - Winifred devises a plan to exact revenge on modern Salem by casting the Magicae Maxima spell, which requires the mayor’s blood. The sisters imprison Izzy and Becca in a basement while forcing Gilbert to gather the necessary ingredients for their dark ritual.

Meanwhile, the girls make their escape to alert the mayor. As the sisters wreak havoc at the town’s Halloween carnival, enchanting the citizens to assist in their search for the mayor, Gilbert resurrects Billy Butcherson - played by Doug Jones - to aid in their nefarious plot despite needing his head for the spell.

The girls reunite with Cassie and attempt to contain the sisters within a salt circle in Cassie’s garage, but their moment of harmony is shattered when the sisters break free and kidnap Cassie to use her blood as an alternative. Izzy and Becca pursue them into the forbidden forest, where Gilbert has gathered the spell components, ultimately revealing that Becca possesses innate magical abilities akin to the witches.

The sisters begin to cast the spell, amplifying their powers, but Becca cleverly distracts them long enough for Izzy to rescue Cassie. In an unexpected turn, Becca manages to convince the enchanted book, known as Book, to forsake its allegiance to Winifred, prompting their escape deeper into the woods. There, they unearth a warning denouncing the Magicae Maxima spell, stating that any who cast it must sacrifice what they hold most dear to gain ultimate power.

Rushing to warn Winifred of this dire consequence proves futile, as she becomes all-powerful while Mary and Sarah disintegrate into dust. Overcome with sorrow, Winifred implores the girls to leverage their newfound powers to save her sisters. Though they are unable to fulfill her wish, Becca, Cassie, and Izzy form a coven and perform a reuniting spell, allowing Winifred to joyfully fade away to be reunited with her sisters in the afterlife.
