Kit Li rushes to a local school where a terrorist group called The Doctor has taken a school bus hostage. He soon learns his wife and son are aboard the explosives-rigged bus, heightening the stakes. The standoff ends in a catastrophic explosion that kills everyone inside the vehicle.

In Hong Kong, Kit has left the police and become a stunt double for martial arts star Frankie Lone. Helen, a tabloid reporter, films one of Kit's stunts and uses Lone's duplicity to boost her show's ratings.

Frankie's father and manager invite Kit to a jewelry exhibition at a newly opened highrise hotel. The Doctor also targets the exhibition, using it as the setting for his escalating plan.

At a traffic stop, Kit overhears the Doctor uttering a catchphrase from the bus bombing. Realizing the villain's identity, he tails the car back to the hotel to confront the threat.

The Doctor's team seizes the hotel during the exhibition and massacres the guests. Kit and Chow arrive and are immediately ambushed by the terrorists, while Frankie manages to escape and encounters Fai.

Kit and Chow manage to drive a car out of the freight elevator, thinning the Doctor's numbers and disrupting the terrorists' control. The fight continues as hostages and rescuers scramble for safety.

Frankie's father wrestles a weapon away from a terrorist and threatens the hacker trying to deactivate the exhibit's security measures. His bold move buys valuable seconds for the rescuers.

Chow is reunited with his girlfriend Joyce amid the chaos, adding a personal stake to the peril faced by the hostages and rescuers.

Driven by the Doctor's taunts, Kit makes a doomed attempt to kill the villain. The act backfires, tipping the balance back in the terrorists' favor and heightening the danger for everyone.

Helen hides in a reptile exhibit to safeguard the footage, but Rabbit releases snakes into the bathroom, poisoning her. Kit and the Lone duo manage to rescue Helen and recover the footage showing the Doctor's face.

The Doctor issues impossible demands, threatening to toss a hostage from the window every ten minutes. Charlie Tso, the first victim, is selected to demonstrate his resolve.

The Lones confront Fai and Kong in a tense argument that escalates into a fight. Fai holds them at gunpoint and returns them to the other hostages, revealing her duplicitous nature.

Kit retrieves the surveillance tape and kills Rabbit during a grenade blast, then escapes into police custody. The police refuse to let Kit return to the hotel and he forces a helicopter escape at gunpoint.

The Doctor orchestrates an ambush on the helicopter and Helen warns Kit. He rams the helicopter into the hotel building, creating chaos and enabling most hostages to escape, though Frankie, his father, Chow, and Joyce are intercepted by Fai and Kong.

On the hotel rooftop, Chow kills Fai and Frankie kills Kong as the dust settles. The Doctor taunts Kit as Helen is held nearby; Kit throws a dagger coated with snake venom from Helen’s wound, fatally wounding the Doctor. He dies in agony, and his body is looted by teenagers. In the aftermath, Frankie plans to turn the incident into a movie, crediting the heroes, while Kit leaves with Helen.
