Years ago, Daniel is walking home with his mother, Alice, and they pause on a park bench. A clown hands Daniel a balloon; a man lunges from the darkness and murders Daniel, injuring Alice. Alice remembers the killer's face clearly, a memory that haunts her in the present.

In the present day, a string of brutal child murders rocks the city. Alice becomes deeply involved because of the personal memories that tie her to the cases. She notices a man seen near each slaying, fueling her suspicions.

Alice shares her suspicions and the eerie pattern with Detective Weiss. Weiss notes the potential lead but questions Alice's reliability. Their uneasy alliance forms as they document clues surrounding the killer.

Alice corners a man named Sam, weapon drawn, and demands answers. Instead of killing him, she takes him prisoner to hear his story. Sam claims to hunt a creature from hell that kills children, called the Hellbreed.

Another child is murdered in front of witnesses after a call alerts the investigators. Witnesses describe the killer as a clown, pushing the case toward a supernatural direction. Alice grows conflicted about Sam's claims and the involvement of a monstrous entity.

Alice learns that it wasn't Sam who killed her son, but the clown, contradicting her earlier memory. The realization challenges her beliefs and motivates her to continue helping Sam. She commits to working with Sam to hunt the Hellbreed.

Weiss digs into Alice's history and reveals a hidden past. Alice was committed to a mental institution in 1989 and escaped in 1995; she is still sought by authorities. The revelation adds tension to the ongoing investigation.

Outside the institution, the Hellbreed appears and kills Detective Weiss. Alice is spared as the monster backs away. Sam arrives and kills the Hellbreed, ending the immediate threat.

The film intercuts with footage of Alice being interviewed after the events, serving as narration. She recounts the events from the institution's perspective, blurring memory and report. The device hints at unresolved trauma in Alice.

The interview shows that Alice has not achieved peace, breaking down emotionally under the camera. Her breakdown underscores the film's meditation on memory and guilt. The screen fades to black as the trauma remains unresolved.
