Orville Kruger brings his daughter Lisa and her friend Kathy to the family's remote country house in the San Bernardino Mountains. They settle in at the isolated residence, unaware that danger is about to intrude on their mountain retreat.

While en route, Scratch and his gang in a Volkswagen van harass the girls and threaten a robbery. The gang has a violent past, on the run after a mass murder in Nevada.

Lisa's mother informs them that Lisa's uncle Rod, a plastic surgeon, will arrive the next day. Orville plays a prank using his special effects to entertain the girls, blurring the line between reality and performance.

Scratch and his gang invade the house, holding the family hostage and demanding valuables. The tense assault signals the beginning of a brutal night of terror.

Orville is bludgeoned to death with a log, and Lisa's mother is killed during the invasion. The violence leaves the survivors overwhelmed and vulnerable.

A pair of eyes appears to observe the brutality from behind a glass slot in a wall, suggesting a hidden menace within the house.

The gang attempts to rape Kathy, but a female member stabs her to death. Lisa manages to escape the house on foot, fleeing into the surrounding darkness.

The hideously deformed Patrick emerges, murders two gang members, and the rest flee into the woods. They discover a hidden room decorated like a nursery, hinting at a dark secret the family kept.

At dawn, Lisa is attacked by a gangster, but Patrick arrives and saves her, killing the attacker. The early light brings a fragile beacon of hope amid the carnage.

Rod arrives at the house and finds police there who suspect Lisa may have killed someone. Lisa is later reported missing, and investigators begin to connect the violence to her.

Authorities locate Lisa unconscious in the woods; Scratch and Shelley claim they were innocent victims. The investigation remains muddled as Patrick's rampage is partially revealed.

After Lisa's death and the chaos, Rod reveals the truth about Patrick's deformity and the Krugers' past. He begins to take matters into his own hands, renting two surgical tables and obtaining a shotgun.

Outside the jail, Rod kidnaps Scratch and Shelley and drives them back to the Krugers' country home. The plan is to subject them to a punishment reminiscent of Patrick's control.

In a makeshift operating room, Rod removes a mask revealing his own facial deformity and then mutilates Scratch and Shelley. He locks them inside Patrick's soundproof nursery to await their fates.

The brutal events are revealed not to be real; they are part of a test screening of a feature film. The screening is interrupted when the Frankenstein Monster and the Wolfman burst in, terrifying the audience.
