A catastrophic Island Line train crash on the Isle of Wight triggers a hospital crisis. St. James's becomes completely full with casualties, forcing Mercy Falls to remain open only as an older, closing facility. Some patients, including the children's ward, are kept at Mercy Falls until there is space elsewhere.

Susan takes sickness leave and Amy Nicholls steps in as the new night nurse. Amy forms an immediate bond with Maggie, who is an orphan like herself. This shift sets the stage for the growing connection between Amy and the children.

Maggie tells Amy that 'she' is coming, hinting at a malevolent presence. The warning unnerves Susan and foreshadows events around the upstairs wards. The eerie hint introduces Charlotte as a looming figure in the ward's legend.

Simon wakes with intense pain in his leg and is taken for an X-ray. The doctors discover a broken femur with no clear explanation of how it happened. The mystery deepens as the ward seems haunted by unseen forces.

Photographs and medical files reveal that Charlotte is the vengeful ghost of Mandy, a girl with osteogenesis imperfecta, and that the nurse was obsessed and murdered her. The nurse dies by leaping into an elevator shaft as the hospital's past bleeds into the present. Amy realizes the upstairs ward is haunted by this unfinished violence.

The ward's rage makes the hospital crumble as evacuation ensues with help from St. James's. Mrs. Folder reports Maggie is missing as chaos spreads. Amy must race to the abandoned ward to save the girl, with time running out.

Amy finds Maggie in Mandy's old room, weakened by her illness and desperate to reach safety. Charlotte's presence destabilizes the floor and causes Amy's leg to be impaled on a piece of metal, leaving her bleeding. They barely escape through a trapdoor, but Maggie dies in Amy's arms and Amy collapses afterward; Mr. Sleepy falls away during the struggle.

Robert and the medical team attempt to revive Amy with a defibrillator, but her heart stalls briefly. Maggie's ghost appears and offers a 'pure love' kiss, reviving Amy as in Sleeping Beauty. The film ends with Amy waking in another hospital with Robert by her side.
