Shin defeats Ken in a brutal duel, leaving him gravely wounded and unable to continue. Shin carves seven wounds into Ken's chest as a symbol of his victory and seizes Yuria, dragging her away from Ken. The betrayal shatters their plans and sets off a brutal power struggle in the wasteland.

Raoh returns to his dojo and confronts Ryuken over the Hokuto Shinken succession. He murders Ryuken in a decisive clash, rejecting the old order. With Ryuken dead, Raoh proclaims his rule over the world and begins his long campaign of conquest.

A year passes while chaos deepens under Raoh's influence. Ken becomes a wandering protector of the weak, striking against predatory gangs and offering hope to survivors. He saves Bat and Lin, two young fighters, and teams up with Rei, another Nanto Seiken master searching for his kidnapped sister Airi.

Ken learns that Airi has been kidnapped by Jagi, a former brother-in-training who impersonates Ken to tarnish his name. He tracks Jagi to his hideout, defeats him in battle, and rescues Airi from captivity. The confrontation links Jagi to a larger scheme threatening Yuria and the city of Southern Cross.

In his dying moments, Jagi confesses that he manipulated Shin into betraying Ken. He hints that Yuria is living with him in Southern Cross, tying the betrayals together. The revelation heightens Ken's resolve to reach Yuria and confront his enemies.

Raoh consolidates power, amassing a massive army and sweeping across the wasteland by defeating rival warlords. His expansion threatens every remaining settlement as he heads toward Southern Cross. The looming invasion intensifies the struggle for Yuria's fate.

In Southern Cross, Shin rules with a firm grip while Yuria resists his control. She longs to be reunited with Ken, even as Shin uses his army to maintain order. The city becomes a focal point in the coming clash between Raoh and Shin.

Kenshiro reaches Southern Cross to find the city engulfed in flames and Shin's forces weakened. Shin fights Ken but is already mortally wounded from Raoh's earlier confrontation and dies soon after, revealing that Raoh has Yuria and is heading toward Cassandra. Ken resolves to pursue them and rescue Yuria.

Lin, Bat, and Rei arrive in Cassandra to witness Raoh's vast army marching through the city. Lin spots Yuria being held in Raoh's dungeon and realizes the danger she faces. They plan a nighttime rescue and to find a way to end Raoh's tyranny.

Rei challenges Raoh but is no match for his overwhelming strength and dies. Kenshiro arrives in Cassandra to confront Raoh, and the two powerhouses unleash their fighting auras, tearing through much of the city and leaving both gravely damaged. The city becomes a brutal battleground as their duel unfolds.

Exhausted and on the edge of defeat, Raoh knocks Ken unconscious, but Lin intervenes and pleads with Raoh to spare his life. After a tense pause, Raoh relents and agrees to postpone their duel, sparing Ken for now. Ken departs Cassandra with Lin and Bat to continue the search for Yuria.

Lin and Bat slip into Raoh's dungeon that night and manage to free Yuria, aided by a plant Yuria had given Ken. The plant's presence catches Raoh's attention, but Yuria is soon sentenced to public execution the following morning as Raoh tightens his grip on the city. The rescue marks a narrow victory but leaves their fate uncertain.

During the climactic battles, Yuria vanishes, leaving Ken with an unresolved lead and a burning need to keep searching. Kenshiro remains determined to reunite with Yuria, even as Raoh's ambitions loom over the wasteland. The story closes on an open note, with Ken's mission continuing beyond the film.
