The story takes place in a post-apocalyptic future after a global nuclear war has devastated Earth. Most of the planet is contaminated and sparsely resourced, forcing survivors to cluster in fortified cities. Between these urban strongholds lies a harsh, contaminated wasteland where only the strongest survive. Warlords control different territories, and the old social order has collapsed entirely.

The primary settings are Southern Cross, a fortified city ruled by Shin where Yuria is held in luxury, and Cassandra, known as the City of Wailing Demons, which serves as an arena for combat between Ken and his rivals. The vast wasteland surrounding these cities connects the locations and tests every traveler's survival skills.

Kenshirô is a master of the lethal martial art Hokuto Shinken who roams the wasteland as a protector of the weak. Betrayed by his former friend Shin and pursued by his rival Raoh, Kenshiro is driven by a personal code of justice, the hope of reuniting with his fiancée Yuria, and a mission to defeat the tyrants who prey on vulnerable survivors.

Raoh is Kenshirô's oldest rival and a powerful conqueror who proclaims himself ruler of the new world. As a master of Hokuto Shinken's rival style, he leads a massive army that clashes repeatedly with Kenshirô. The struggle between them represents the central conflict over who will shape the future order of the post-apocalyptic world.

Yuria is Kenshirô's fiancée who longs for his return. She is currently held captive in Southern Cross under Shin's control.
