In 1968, Iris Campbell and her fiancé, Paul, attend the grand opening of the Skyview Restaurant Tower. The event spirals into chaos as Iris experiences a disturbing premonition: she envisions a shard from the chandelier shattering the glass floor, leading to a catastrophic explosion that causes the tower to collapse, resulting in the deaths of all the guests present.
Fast forward to the present, the college student Stefani Reyes begins to suffer from haunting nightmares related to Iris’s premonition. Seeking clarity, she returns home to her family, where she is greeted by her father Marty and her younger brother, Charlie. Together, they pay a visit to their relatives, including Uncle Howard and Aunt Brenda, alongside cousins Erik, Julia, and Bobby. When Stefani inquires about her grandmother Iris, Howard reveals that Iris distanced herself from the family, prompting her mother, Darlene, to leave as well. As a token of their past, Brenda gifts Stefani letters Iris wrote to Howard, prompting her to venture to Iris’s secluded cabin in the woods.
Upon reaching the cabin, Iris explains the drastic impact of her warning to Paul and the guests that fateful day: she inadvertently disrupted Death’s design, resulting in the tower never collapsing. However, Death’s vengeance ensued as it began to stalk the survivors and their descendants, ensuring they meet grim fates through a series of deadly accidents. Paul did not escape these tragedies and became one of the many casualties of the ensuing chaos. Iris had dedicated herself to documenting omens in a book, desperately trying to outrun Death’s grasp and protect her family. Tragedy strikes as a freak accident sends a weather vane impaling Iris, ending her life abruptly.
At her funeral, tensions flare as Darlene returns home, with Stefani harboring resentment over her mother’s absence during their childhood. A family barbecue unfolds, and while reading Iris’s book, Stefani discovers mentions of a survivor named J. B. who had eluded Death. During the family’s celebrations, a horrifying chain of events occurs: Howard steps on glass and meets a gruesome fate with a lawn mower, catalyzing a realization that Death is systematically targeting the family following Iris’s death. Despite the danger, Marty and Brenda remain unscathed, as they are not part of Iris’s bloodline.
Darlene suggests they consult William Bludworth, now working in a hospital, who had survived the original tower collapse. Bludworth recounts his childhood experience and shares that only “new life” has the power to defeat Death, mentioning that previously, survivors Kimberly Corman and Thomas Burke had found a way to do so. He reveals his own terminal illness, awaiting Death’s arrival, intensifying the family’s sense of urgency.
In a desperate bid to generate new life, Erik and Bobby devise a reckless plan involving Bobby inducing an allergic reaction, aimed at creating a life-and-death scenario that Erik could potentially reverse. The attempt backfires horrifically, leading to brutal accidents in the hospital that claim both Erik and Bobby’s lives. Meanwhile, Stefani, Charlie, and Darlene attempt to escape to Iris’s cabin in their RV, only to suffer a deadly crash. As they grapple with the fallout, Darlene heroically saves Charlie but is tragically bisected by a falling lamp post. In an intense moment, Charlie manages to free Stefani from her seatbelt and revives her just in time, inadvertently thwarting Death’s plan.
One week later, at Charlie’s graduation ceremony, the siblings discover a chilling truth: a person is declared dead from drowning only after five minutes. With Stefani revived within a minute, hope flickers for their future. However, just as they grasp the possible implications of their survival, disaster strikes once again as a train derails, sending heavy logs crashing down upon them, sealing their fates once and for all.
