As children, Jonesy, Beaver, Pete, and Henry discover they share telepathic powers after saving Douglas 'Duddits' Cavell. They nickname the power 'the line' and form a strong bond with Duddits. This early connection foreshadows the danger and connection that resurface during their hunting trip.

One night, Duddits beckons Jonesy to cross the street and he is hit by a car. The injuries heal with mysterious speed, establishing the supernatural link among the friends. Six months pass before the group reunites for their annual hunting trip.

During the trip, Jonesy rescues Rick McCarthy, a man lost in the forest who is gravely ill. Jonesy and Beaver bring him to the cabin to rest and recover. The recovery is interrupted by the ominous signals of quarantine sweeping the area.

That night, forest animals flee in the same direction as two military helicopters announce a quarantine. Rick is found dead in the cabin, and a three-foot worm-like creature writhes in the toilet. Beaver is killed when the creature escapes, and Mr. Gray begins possessing Jonesy.

Mr. Gray uses Jonesy’s body to access memories of Duddits, tightening his control over the situation. Jonesy struggles against the possession as the others become aware something has invaded their friend. The danger grows as the possession reveals more about the alien threat.

Henry and Pete crash their SUV to avoid a frostbitten woman from Rick's party. Henry goes for help while Pete stays with the woman; she dies and also excretes a worm that Pete kills. Mr. Gray abducts Pete, and Jonesy warns Henry to stay hidden.

Beaver's death and the larvae spread; cabin fire

Henry returns to the cabin to find Beaver dead and a group of eggs laid by the worm. He burns the cabin in an effort to destroy the larvae and prevent further infection. The forest outbreak continues to threaten everyone nearby.

Curtis's operation and the air strike

An elite military unit led by Colonel Abraham Curtis arrives to contain those exposed to Ripley, intending to retire after the operation and pass his pistol to Captain Owen Underhill. They mount an air strike on the aliens’ forest clearing, destroying most of them with miniguns and missiles. The alien ship self-destructs, destroying the remaining aliens and two helicopters.

Henry and Underhill confront the quarantine plan and head toward Duddits' home

Henry reaches the quarantined camp and realizes Curtis intends to kill everyone to prevent further infection. He convinces Underhill to override the plan, and they escape toward Duddits’ home to learn more. Duddits, dying of leukemia, warns that Mr. Gray is headed for the Quabbin Reservoir.

Warning and the journey to the reservoir

Duddits informs Henry and Underhill that Mr. Gray intends to seed the Quabbin Reservoir with alien larvae to infect the world. They resolve to reach the reservoir and thwart the plan. Curtis continues to pursue them, fueling the chase.

Showdown at Quabbin and the explosive climax

At the Quabbin Reservoir, Henry, Underhill, and Duddits confront Mr. Gray as Curtis's forces close in. Underhill is mortally wounded and Curtis is killed during the assault. In the reservoir's pump house, Mr. Gray exits Jonesy's body to infect Duddits with Ripley, and the two aliens explode in a red-dust cloud as Jonesy steps on the final larva.
