A woman navigates her way through a dimly lit parking garage toward her car. Once inside, she encounters difficulties starting it, and soon three menacing thugs appear. They break into her vehicle, forcibly pull her out, and attempt to assault her. Just when it seems hopeless, Paul Kersey (Charles Bronson) emerges unexpectedly, shooting down the first two assailants, though he only manages to wound the third. As the remaining thug tries to make a getaway, Kersey quickly blocks his escape route. The desperate criminal begs for mercy, but Kersey shows no hesitation, ending his life with a shot. In a shocking twist, he rolls the thug over with his foot to reveal that the entire assault was merely a nightmare, and Paul awakens in a panic, drenched in sweat.
The following morning, a young woman named Erica (Dana Barron) visits Paul’s architectural firm in Los Angeles, and it’s revealed that she is dating Paul’s partner, Karen Sheldon (Kay Lenz). Later that evening, Erica goes to an arcade with her boyfriend Randy Viscovich (Jesse Dabson), where they meet up with Jojo and his friend. Unbeknownst to Erica, Jojo offers her drugs, leading to her tragic overdose. The next night, driven by a mix of concern and anger, Paul tracks Randy to the arcade after learning of Erica’s fate. Randy confronts Jojo about the drugs, threatening to notify the authorities. In a bid to silence him, Jojo ruthlessly kills Randy, but Paul retaliates by shooting Jojo, who falls onto the electrified bumper car ride.
Upon returning home, Paul receives a call from Nathan White (John P. Ryan), a publisher who knows about Jojo’s death. Nathan shares his own heart-wrenching story of loss, explaining how his daughter fell into addiction and ultimately died from an overdose after his wife passed away. He proposes a proposition to Kersey: eliminate the drug trade plaguing L.A., specifically targeting rival drug lords Ed Zacharias (Perry Lopez) and Jack Romero. After some contemplation, Paul consents to Nathan’s request, armed with new weapons and intel to dismantle the drug empire.
Paul skillfully infiltrates Zacharias’s opulent mansion during a lavish birthday celebration. While bugging Zacharias’s phone, he inadvertently witnesses the drug lord murder a colleague for betrayal. Captured, Paul is coerced into helping dispose of the body, but he seizes the opportunity to defend himself by killing his captor. This sets off a chain reaction, as Paul goes on to eliminate several of Zacharias’s associates in various high-stakes confrontations, each one intensifying the war between Zacharias and Romero’s factions.
As the body count rises, Paul Kersey becomes a persistent shadow haunting the operations of both crime families. Eight more of Zacharias’s men meet a grisly end at the fisherman’s wharf, while corrupt officer Phil Nozaki (Soon-Tek Oh) tries to uncover Kersey’s identity, only for Kersey to end him in self-defense. Meanwhile, Detective Reiner (George Dickerson) vows to hunt down Kersey, unaware of Nozaki’s betrayal.
In a climactic retaliation, Zacharias and Romero agree to meet, thinking they are facing each other, but Kersey uses this opportunity to eliminate both adversaries. Afterward, Nathan praises Kersey for his ruthless efficiency, but soon reveals his true motives. When Kersey falls into an ambush, narrowly escaping an exploding limo after recognizing the set-up, he becomes a rogue man on a mission seeking justice.
What follows is a pulse-pounding confrontation between Kersey, Nathan’s impostors, and the remaining thugs in a high-stakes chase. As Paul traverses through death traps and adversaries, he faces off against an injured Karen, torn between his quest for vengeance and the love of his life. In the final showdown, Kersey eradicates Nathan’s forces, but not before tragedy strikes again as Karen is fatally wounded. Fueled by rage, Kersey exacts revenge on Nathan, before confronting Reiner, who is left bewildered by Kersey’s survival. Kersey’s chilling conclusion echoes softly as he tells Reiner, “Do whatever you have to,” before walking away into the night, leaving a path of destruction behind him.
