Andy Barclay has kept the severed head of Chucky and tortured it for four years, using it as a twisted fixation. The film opens by establishing his unstable obsession and its dangerous consequences. The head remains a sinister reminder of the doll's ongoing influence over him.

Nica Pierce remains in a mental institution after being framed by Chucky for the murders of her family. She believes she is responsible and that Chucky was only a figment of her imagination. The oppressive environment heightens her struggle to trust her own memories.

Tiffany Valentine visits Nica as the legal guardian of her niece Alice and delivers grim news: Alice has died. She leaves behind a second Good Guy doll, which is possessed by Chucky's spirit. The presence of the doll signals the doll's ongoing malevolent influence.

That night, Chucky awakens and discovers that Nica has attempted suicide by slit wrists. The next morning, Nica finds her wrists stitched up, Angela is dead, and she realizes that her delusions about Chucky were real. The dolls' return marks a turning point in her perception of reality.

Nica learns that Chucky has killed Claire within the facility. The murder confirms that the doll's influence extends beyond a single host and signals a broader threat. Fear and confusion spread among the patients as the staff scramble to understand the danger.

Malcolm is killed by one of the dolls after admitting that he's not truly Charles. The third Chucky unlocks Andy's cell and attacks him, leading to a tense standoff. Andy rips open the doll's chest, pulls out a hidden gun, and shoots it, but cannot escape before Nica-Chucky traps him back in his cell.

Nica-Chucky frees itself and reunites with Tiffany. They drive off together, accompanied by a Tiffany doll that is alive and shares a portion of Tiffany's soul. The scene sets up a cliffhanger ending with multiple Chucky vessels on the loose.

In the post-credits scene, Kyle, Andy's former foster sister, enters Andy's house to continue torturing the severed Chucky head.
