Danielle Bowden (Juliette Lewis) provides a haunting narration about her trepidation towards the Cape Fear River, revealing her desire to escape the dark shadows of the past. This tumultuous past is trigged by the release of Max Cady (Robert De Niro), whose vengeance-fueled tattoos tell a chilling story. Meanwhile, Danielle assists her mother Leigh (Jessica Lange) with their home business while her father Sam Bowden (Nick Nolte) interacts with Claude Kersek ([Joe Don Baker]) as he exits the courthouse. Their lives unexpectedly collide at a local movie theater, where Max makes a scene and smokes defiantly.
Sam finds solace playing racquetball with Lori Davis ([Illeana Douglas]), but his worries about his public image plague him. Max soon confronts Sam, reminding him of his imprisonment, which he directly blames on Sam. In a desperate attempt to safeguard his family, Sam proposes a payoff to Max, but to his dismay, Max declines, revealing his knowledge of the law acquired during his time in prison. Tensions escalate as Sam discovers that his dog has been poisoned, raising his suspicions towards Max without any tangible evidence.
Leigh’s anxiety heightens when she witnesses Max spying on their home at night. In a conversation with Lieutenant Elgart ([Robert Mitchum]), Sam remains vigilant about Max’s predatory behavior. Despite a physical confrontation, Max remains unperturbed. As Max stalks Lori at a bar and subsequently attacks her, Sam learns of the incident and visits Lori in the hospital, where she is too traumatized to testify against her assailant. When Sam confides in Kersek about trapping Max, he flatly refuses, further straining his relationship with Leigh about the looming threat they face.
Max, undeterred by threats, begins communicating with Danielle under the guise of her drama teacher, leading to a kiss that complicates matters further. In response, Sam organizes an ambush on Max, but Max turns the tables on Sam’s hired thugs, showcasing his cunning. Max retains lawyer Lee Heller ([Gregory Peck]) to defend him, bringing to light a tape recording of Sam threatening him, which leads to a restraining order being issued against Sam. Realizing the true depths of Max’s menace, Sam seeks a weapon, indicate his growing desperation.
The storyline escalates when Kersek devises a trap for Max. Sadly, the plan goes awry as Max outsmarts Kersek, leading to his demise.
