Ethan McCoy, played by Nick Nolte, is a DEA agent who finds himself amidst chaos as he investigates a charred house in search of John Cain, portrayed by Josh Duhamel. Just as he orders his team to locate Cain, the latter escapes just moments before the house erupts in flames, pursued by armed foes. With his car obliterated, Cain awakens in a Mexican hospital with no memories of his past. By his side is a woman named Anna, played by Abbie Cornish, who claims to be his wife, alongside a friend named Eddie, portrayed by Omar Chaparro.

Eddie assures Cain that he will help restore his memories, even resorting to threats against the doctor in charge of Cain’s care. As Anna reveals his birth date, Cain’s memory begins to return in fragments, leading him to suspect that Anna might not be who she claims. Realizing every recollection triggers the doctor’s sedative, Cain reacts violently, attempting to flee with Anna amidst Eddie’s relentless pursuit.

Inside the hospital, tensions rise as Cain learns that Eddie, a drug dealer, is deeply entangled in his past. Struggling with the truth, Cain confines Anna to a chair, convinced she is deceiving him. After a frenzied confrontation with Eddie’s men, he discovers clues about a mysterious case he possessed before falling unconscious. This case, stolen from General Mondejo—a powerful figure within the drug cartels—holds secrets that everyone is desperate to retrieve, leading to a tangled web of deceit.

In a critical moment, Cain contacts McCoy, who discloses his undercover role within Eddie’s organization. With revelations about the case’s content—detailing the names and addresses of influential figures—Cain grapples with his identity. As Anna insists that he was recruited by the CIA for a mission, the lines between hero and villain blur.

In pursuit of the truth, Anna manages to recover the case, which holds sensitive information that could destabilize global order. As Cain recalls their romantic history, he battles the remnants of Eddie’s crew, only to face yet another twist: McCoy, aligning himself as another adversary, has his own ambitions for the case.
