Eight years after taking down the notorious drug lord Fouchet, detectives Mike Lowrey and Marcus Burnett resume their relentless fight against crime in Miami. They focus their attention on a new drug trafficking operation involving ecstasy that has become rampant in the city.

Mike and Marcus attempt to infiltrate a Klan meeting, hoping to uncover information about the drug trade. To their surprise, they find out that the Klan members are buyers of the drugs rather than distributors, leading to an unexpected confrontation.

A radio malfunction leaves the Miami Police Tactical Narcotics Team delayed, forcing Mike and Marcus to face the Klansmen alone. Amidst the chaos, Mike accidentally shoots Marcus in the buttocks, complicating their operation and further straining their partnership.

Following the gunfight, Marcus begins to reconsider whether he should continue working with Mike. His internal conflict is compounded by the fact that Mike is secretly dating Syd, Marcus' younger sister, leading him to contemplate transferring out of the Tactical Narcotics Unit.

Unbeknownst to Marcus and Mike, Syd is revealed to be working undercover for the DEA. She is posing as a money launderer for a branch of the Russian mafia led by drug lord Hector Juan Carlos 'Johnny' Tapia, significantly complicating the situation.

Syd attempts to transport a large amount of laundered money when she is intercepted by a gang from Zoe Pound. As the situation escalates, Mike and Marcus, who had been tracking the gang, quickly get involved to rescue her.

A high-octane gun battle ensues involving Mike, Marcus, and the Haitian gang. The chase through Miami leads to significant collateral damage, eventually resulting in a reprimand from Captain Howard for the chaos created during their operation.

After interrogating the Haitian gang leader, Mike and Marcus learn that Tapia is using a local mortuary as a cover for his drug operations. To investigate further, they disguise themselves as pest exterminators to gather intel.

Mike and Marcus successfully infiltrate the mortuary, making a shocking discovery that Tapia is using cadavers and coffins to smuggle drugs and money. This crucial evidence is pivotal in planning a coordinated raid on Tapia's operations.

Armed with substantial evidence, Captain Howard authorizes a raid on both Tapia's residence and the mortuary. The operation involves collaboration with the DEA and the U.S. Coast Guard, aiming to dismantle his drug empire.

Despite the successful raid where most of Tapia's drugs and money are seized, he manages to escape to Cuba with Syd as a hostage. He discovers Syd's true identity as a DEA agent during the raid, leading to a tense hostage situation.

In a desperate bid to rescue Syd, Mike and Marcus gather a black-ops team comprising DEA agents, TNT members, and an insider from the CIA. They plan to launch an operation directly in Cuba, utilizing local resistance contacts for support.

The team successfully infiltrates Tapia's mansion, rescuing Syd from his clutches. In a final act of vengeance, they leave behind an explosive device that demolishes the mansion as they make their dangerous escape.

While fleeing in a stolen Hummer, Mike, Marcus, Syd, and their ally Tito face dangerous pursuit from Tapia. Their escape leads them to a tense standoff with U.S. Navy personnel at Guantanamo Bay, who mistakenly open fire on them.

In an ultimate confrontation, Marcus bravely takes down Tapia just as he holds Mike at gunpoint. With Tapia dealt with, the trio celebrates their survival and partnership at a family barbecue in Marcus' backyard, solidifying their renewed bond.
