As Ava’s (Louisa Krause) grip on reality begins to stabilize after a harrowing 28 days of possession by a malevolent entity named Naphula, she faces the stark aftermath of her ordeal. With the guidance of a priest (John Ventimiglia), she emerges from the shadows of her traumatic experience, only to find herself grappling with significant loss: her job, friends, and boyfriend are all gone, leaving her with a mountain of consequences and the looming threat of imprisonment.
In the wake of her release, Ava’s family circles around her for support. Her parents, William Sadler and Deborah Rush, along with her sister Whitney Able and her sister’s fiancé Roger (Zachary Booth), implore her to consider a plea deal. This agreement could keep her out of prison if she agrees to participate in a rehabilitation program tailored for those recovering from possession experiences.
Under the strict supervision of a no-nonsense facilitator named Tony (Wass Stevens), the program aims to equip participants with tools to build emotional resilience and fend off future demonic influences. A pivotal element of the rehabilitation includes wearing a necklace that, paradoxically, symbolizes an invitation for the demon’s return. Participants must resist this charm’s allure; success means they possess the necessary strength to avoid future threats and ultimately graduate from the program.
As Ava traverses this challenging new path, she forms an unexpected connection with Hazel (Annabelle Dexter-Jones), a fellow program member who surprisingly cherishes her past connection with the same demon that haunted Ava. Hazel’s obsession with Naphula complicates Ava’s healing, igniting unease as Ava struggles to piece together fragmented memories of her possession. A shocking moment occurs when she discovers a bloodstained trail leading through her home, intensifying her suspicions that her family is hiding critical information.
The only clue is an intricately engraved timepiece that directs her toward Ben (Lou Taylor Pucci), who reveals that it belonged to his father. Despite initial hesitations, Ava forges a fragile bond with Ben, unaware of the storm brewing around them. However, a traditional ritual she embarks upon to reconnect with Hazel’s spirit leads to disastrous outcomes. It inadvertently lands Hazel in a mental institution, while Ava is left with a cryptic tattoo on her neck that Tony later discovers. This marks a turning point, as Tony fears the tattoo could reignite the demon’s influence, leading to Ava’s expulsion from the program.
In a desperate bid for her freedom, Ava’s family attempts to commit her, only for her to uncover the terrible truth about Ben’s father. Contrary to her assumptions, he was not a victim but rather a hitman hired by Roger to silence any threats to their sinister dealings. As she pieces together Roger’s dark motivations, Ava finds herself caught in a perilous confrontation where Roger, fueled by his own malice, implores her to wear the necklace once more to revive the demon’s power.

However, beneath the surface, remnants of the demon’s presence linger ominously, intertwining with her daily tasks. The mundane becomes extraordinary when Ava inadvertently triggers a series of falling boxes, revealing a cache of ancient files chronicling others’ encounters with the same dark force. This discovery ignites a troubling realization: her mother was deeply entwined in similar dark tales, suggesting a sinister legacy that is, perhaps, woven into their very bloodline.
As troubling memories of her sister resurface, particularly one involving a strikingly familiar tattoo on her sibling’s neck, Ava grapples with the chilling possibility that her own sister may have cast this darkness upon her, desperate to escape its grasp. Driven by betrayal and fury, Ava’s emotional crescendo erupts into a primal scream, breaking through the barriers of her office space, allowing the shadow of the demon to once again assert its presence, a stark reminder of the unearthly forces that continue to ravage her life.
