On a stormy night drenched in rain, the quaint town of Fairwater is shaken by a brutal encounter between a woman named Patricia Bradley (Dee Wallace) and a shadowy figure lurking in her home. In sheer terror, Patricia screams for her life as her mother, armed with a pump-action shotgun, emerges from the shadows and fires at the looming shape that threatens them.

Amidst this chaos, townsfolk converge for a funeral while local newspaper editor Magda Rees Jones (Elizabeth Hawthorne) and her assistant investigate yet another inexplicable death in their community. Enter Frank Bannister (Michael J. Fox), a self-proclaimed psychic investigator, who shamelessly tries to peddle his ghost-busting services amidst the mourning crowd. As he exits in his dilapidated Volkswagen, he faces an accident that leaves one disgruntled neighbor, Ray Lynskey (Peter Dobson), infuriated after his beloved garden gnome meets an untimely end.

On the other side of town, Doctor Lucy Lynskey (Trini Alvarado) pays a visit to the Bradley residence to attend to Patricia’s injuries. Upon seeing the young woman’s fragile demeanor and the suspicious bruises around her neck, Lucy becomes concerned as Patricia’s mother hastily dismisses the need for medical attention, hinting at an ominous past. Curious about Patricia’s history, Lucy starts to watch a documentary on the infamous serial killer Johnny Bartlett (Jake Busey), who was wedded to a spree of horrific murders at the Fairwater Sanatorium long ago, suggesting connections that intertwine the fates of Patricia and the killer.

Just as Ray and Lucy contemplate their wedding plans, an uncanny incident flips their world upside down. Objects levitate around their bedroom—a sign of supernatural presence that plagues the town. Frank arrives to clear the house of spirits, but a glowing number appears on Ray’s forehead, indicating deeper, darker forces at play.
