In a bleak coastal world where desperation gnaws at the edges of daily life, a tight-knit trio of heroin addicts—Cesare, Enzo, and Ciopper—scrapes together money for their next fix, sliding through the days with a shared hunger and mounting risk. Cesare clings to the hope that his girlfriend, Michela, might help pull them out of the spiral, but she’s already pulled into the drug trade herself, buying from Teresa, a prostitute from Ostia, and the two quickly clash in a heated argument that betrays their fragile bond.
The trio manages a score from a local pusher, who goes by the nickname Dartagnan, but the fragile equilibrium shatters when Michela tests Teresa’s heroin and discovers it’s been ripped off. She urges Cesare to demand restitution, yet Cesare hesitates, and Teresa’s pimp—Pimp—steps in to shield her. The tension unfolds into a new day’s plan: Michela, Cesare, Ciopper and Enzo ride a train to Rome to reach a methadone clinic, where they encounter a broader circle of friends and misfits.
Among them are Donna, a trans woman, her partner Debora, and Massimo, a fellow addict freshly out of prison who pushes Cesare toward a risky burglary. The plan fails to deliver, and Cesare hands over the looted money to Massimo to keep the drug scores flowing. On the sidelines, Enzo crosses paths with Mario, a local pusher who fancies a young heroin-addicted girl named Loredana. Seizing a moment of vulnerability, Enzo secures four doses with the barter of future profits, but the gains curdle into tragedy as he ends up shooting all four doses himself.
The night grows denser with encounters: the group visits Patrizia, a local artist who opens her studio to them, sharing her stash as they drift through a fog of intoxication. They spend the hours in the artist’s space, collapsing into drugs as the hours slip by. Eventually, Cesare and Michela drift apart from the group and return to Ostia, where they start to confess a possible future beyond the needle’s grip. They vow to kick their addiction, and to commemorate the moment, they indulge in one final, cocaine-fueled injection before Michela unexpectedly overdoses.
Cesare rushes Michela to the hospital, and while guilt and anxiety coil inside him, he returns to the beach and uses the last of their supply before racing back to the emergency room, where a doctor fights to save Michela’s life. A police car pulls up, orders him to stop, and when he resists, the officers fire at him. The film closes on a stark montage, flashing back to the initiation of Enzo, Michela, and Cesare into the world of drugs during their teenage years, a haunting reminder of how far they’ve fallen and how little there is to redeem them.
