At the heart of the narrative is a trove of more than 800 videotapes recovered from a quiet house in Poughkeepsie, each one a visual record created by the killer, Edward Carver, chronicling his crimes from abduction to the grisly post-mortem details. Carver is careful to keep his face off camera whenever possible, always disguising himself, which pushes investigators to piece together his whereabouts and the fate of his victims from the raw footage and the people who encountered him.

The earliest tapes depict Carver abducting, raping, and murdering an eight-year-old girl, a graphic opening that sets the disturbing tone for what follows. After this initial crime, he grows more methodical: he lures a couple, the Andersons, into giving him a ride, subdues the man with violence, and chloroforms the woman. He performs a horrific C-section, placing the severed head of the husband inside the woman’s womb before sewing her up and waking her to witness her own nightmare on film. In another clip captured at a gas station, Carver even uses sign language to indicate where the bodies are hidden, underscoring how he manipulates both victims and witnesses.

Carver’s next target is Cheryl Dempsey, Stacy Chbosky, a teenage girl whose boyfriend, Tim Surrey, David Haack, becomes entwined in the killer’s brutal project. Cheryl is imprisoned in a basement, subjected to prolonged abuse—sexual, physical, and psychological—as Carver treats her as his “slave.” Her mother Victoria Dempsey, Lisa Black, makes a televised plea to the kidnapper, a heartbreaking attempt to reach her child and stop the violence, while the investigation races to connect the dots across the footage.

As the body count climbs, Carver shifts his tactic, widening his target to prostitutes and adopting the persona of a policeman, earning him the moniker “Water Street Butcher.” The case increasingly centers on Officer James Foley, Bill Bookston, a police officer whose past includes troubling associations and alleged connections to prostitution, leaving investigators with questions about his reliability. Foley is ultimately convicted and sentenced to death in 1996, but he maintains his innocence and resists plea deals. He is executed by lethal injection on September 9, 2001, and days later a map linked to the case surfaces in his mailbox, hinting that the real killer exploited Foley’s identity to misdirect investigators. Foley is posthumously exonerated on September 12, yet the public attention is dampened by its proximity to the September 11 attacks.

A subsequent breakthrough leads to Cheryl being found in Carver’s empty house. She is rescued but remains profoundly damaged—malnourished, scarred, and emotionally frayed. In interviews, she identifies with her captor and even defends him, insisting that he loved her, a testament to the depth of her trauma. Within weeks, Cheryl dies by suicide, and her body is exhumed after burial, where a tape is found inside the coffin, deepening the mystery surrounding Carver and the reach of his crimes.

The investigators confront the possibility that Carver is still out there, watching the documentary itself, and that more victims may be at risk. The film closes with a stark, unsettling postscript: a brutalized woman is filmed by Carver, who ominously promises to spare her life as long as she does not blink; the moment she defies him, the screen cuts to black, leaving the audience with a chilling, unresolved sense of danger.
