Simon Cable wakes in a hospital in 2002 with no memory of the past two years. Dr. Newman explains that he briefly died and is suffering memory loss, setting up a puzzle of two timelines he must navigate.

A distressed Clair visits Simon, but when he follows her, hospital staff insist no one was there. The moment deepens the sense that something in his memories is unreliable.

Dr. Newman quietly helps Simon piece together fragments of his past, guiding him through memories that seem split between two timelines. The process emphasizes careful progress rather than dramatic revelations.

Simon learns that his brother Peter died before he met Anna, contradicting his belief that Peter was alive when last seen. This revelation destabilizes the sense of what is real across the timelines.

An orderly escorts him for an MRI, but an intruder injects him with an unknown substance during the procedure. He awakens to find himself in the year 2000, starting a second timeline.

He awakens in 2000, in a hospital where a different doctor, Dr. Truman, explains that he was admitted after a car accident and that he has no wife. The new timeline presents a different set of relationships and memories.

In the 2000 timeline, Simon meets a patient named Travis and recognizes Nurse Clayton, whom he had seen in his 2002 dream. He also learns Clair was originally engaged to Peter, and that Anna nurses at the hospital and uses a recording of Peter accusing Simon to blackmail him into marrying her.

Back in 2002, Dr. Newman dismisses Simon's visions as hallucinations caused by anti-anxiety meds. Dr. Truman is no longer at the hospital, but Travis remains on the fifth floor awaiting surgery.

When the 2000 timeline returns, Simon thinks he spots the same intruder from the MRI and stabs him, only to realize the attacker is Travis. He escapes back to 2002, where Travis is unharmed, but the stab wounds suddenly appear and kill him.

A new memory surfaces: Simon recalls pushing Peter out of a window during a fight, then attempting to dispose of the body by rolling it off a cliff. He was caught in the act and forced to flee.

Determined to alter the past, he returns to the night at the family home and confesses love to Clair, but he walks away to protect Peter. Clair pursues Peter as he leaves, and Simon still pushes him out the window, though this time he takes Peter to the hospital.

While driving, Simon is tailgated and Peter briefly regains consciousness, distracting him into a collision that bangs him into another car and kills Clair. EMTs arrive, including a woman who resembles Anna, and Simon hears familiar words from an EMT before blacking out.

He finds himself back in the family home with Peter present, and Dr. Newman urges him to let go. He refuses, clinging to the possibility of another chance, and the scene shifts back toward the hospital where he dies.

The final moment returns him to the hospital bed, dying, and the scene resets to the start of the movie. Simon wakes again in 2002, trapped in a looping memory he cannot escape.
