During the Cold War, a crew of Soviet and British looters push into an abandoned 13th‑century abbey hidden in the Carpathian Mountains. They prepare a dynamite charge to break through the floor, but the ground Torah splits beneath them, dropping the group into a vast, unseen cave system. They press on in the dim corridors, hoping to find a way out, while eerie rattling sounds drift through the darkness.

In the present day, a team led by Dr. Nicolai, with his associate Dr. Kathryn Jennings and a quick‑thinking cameraman, Alex Kim, continues the excavation at the old abbey site. Their probes reveal mysterious mosaics and an enormous river winding through the underground world. Local biologists are intrigued by the possibility of an undiscovered ecosystem, and Nicolai hires a team of divers to explore the depths. The expedition is led by the McAllister brothers, with Jack McAllister and Tyler McAllister at the helm, joined by a crew that includes a skilled rock climber, a careful first scout, a sonar expert, and a seasoned survivalist.

Once they dive into the underground river, the team faces a cascade of peril. Briggs, the first scout, heads ahead and reports back before the feed cuts out, prompting the group to push onward. A sudden attack by a large creature traps Strode, dragging him away and sending a panic through the group. A rock fall follows a ruined water scooter crash, forcing everyone to chase a new route while they search for an exit that seems increasingly unlikely to appear. With no rescue planned for days, the divers must rely on each other as the tunnels twist and flood around them.

As they descend, Kathryn studies the strange life forms and discovers a parasitic mutation shared by every cave creature. She explains that these species likely originated above ground and have, over countless generations, adapted to the subterranean world. The parasite appears to be a native, evolving force, possibly engineered by the cave’s isolated environment. The group finds remnants of earlier expeditions, reminding them that they are not the first to face these chambers and its hidden dangers. They push deeper through rapids, hoping to reach a surface route.

Tragedy intensifies when Nicolai is seen clinging to the cave wall, only to be swept away by a powerful current. His scream echoes through the tunnel as Jack attempts a rescue, but Nicolai is pulled under and disappears. The infection from the parasite begins to alter Jack’s body, mutating him as he endures scratches that spread through his system. A winged creature attacks Charlie, killing him as the struggle for survival grows ever more personal.

With the parasite mutating the explorers, trust frays and the group splits. Some decide to press on with Kathryn and Alex, while Jack, Top, and Tyler stay together, seeking a risky escape route. They discover a cavern scattered with human remains and a passage that might lead to the outside world. But the creatures descend, steal the rebreathers, and close off the route. In a desperate bid, the survivors race toward the river and the shifting passage, but more lives are claimed as Alex is snatched from above. In a final act of sacrifice, Jack triggers a cave‑in to block the way for the pursuing creatures, buying time for the remaining teammates to reach safety.

Weeks later, Tyler and Kathryn meet in a café on the surface.
