In the Cartoon Dimension, Doctor Carnage—a mad scientist with a penchant for brutality—conducts a brutal experiment on a man, disemboweling him and ripping his skull out through his stomach in a gory, cartoonish display of power. Back on Earth, the world shifts between innocence and menace as Cindy and her sister Candy are left to fend for themselves with Cindy’s friend Amy in the house. The mood quickly sours as Cindy and her crew summon Rick and Eddie, hoping for a normal evening, while Candy quietly sinks into a Terror Toons DVD she received in the mail—a sinister, self-contained nightmare created by Satan himself. The DVD introduces Doctor Carnage and his brutal accomplice, Max Assassin, a stolen lab monkey mutated into a monstrous gorilla, turning the film into a nightmare that blurs the line between fiction and vi0lence.
As the friends pass the time with a reckless game of Strip Ouija, Candy dozes off, only to wake as Carnage and Max materialize in her bedroom. What follows is a string of merciless shocks: Candy’s spine is ripped out in a single, mechanized motion; Tommy, a visiting friend, is beheaded when he arrives; a pizza delivery man is dismembered with a giant pizza cutter; and a hypnotic disco number by the villains makes Eddie vomit up his own innards. The mood of forced laughter and fear intensifies as Cindy, Amy, and Rick attempt an escape, only to find that every door has been replaced by vertigo-inducing spirals that twist reality into a claustrophobic maze.
Rick is seized and subjected to brain experiments by Carnage and Max, while Cindy and Amy are ripped apart from one another when a police officer—Police Chief—released from Terror Toons is blown up by a dynamite stick hidden inside a box of donuts. The nightmare escalates as Cindy is captured by Carnage; together with the lobotomized Rick, she is forced to witness Carnage and Max perform a grisly magic act that ends with Amy’s dismemberment.
From here, the terror takes a more surreal turn: the villains drag Cindy to a cartoon version of Hell and present her to the Devil, who lays out a chilling plan to unleash Terror Toons on Earth and corrupt the minds of children. In a moment that crystallizes Cindy’s resolve, she realizes that in cartoons, “anything is possible,” and she embraces a new, superheroic identity to confront the cosmic threat. > anything is possible
Armed with newfound powers, Cindy returns to her home and discovers a machine secretly producing Terror Toons DVDs. Max lunges at her, but she fights back with steel and nerve, snapping his neck and stamping out his brain as a grim, cartoonish end. Carnage charges with a giant axe, and Cindy uses the weapon to sever his head, watching tiny monsters spill from the split skull in a macabre, unhinged finale. She then smashes the Terror Toons DVD press with a crowbar, breaking the machine that fed the nightmare to the world.
As the credits creep closer, Cindy and Candy’s parents arrive home from a wedding to a horrifying scene: Rick is banging his head against a wall while Cindy laughs manically, surrounded by the remains of her friends and sister. Nearby, a neighbor child discovers another copy of Terror Toons in the mailbox and rushes inside with it; the front door slams shut behind him, and Carnage’s giggle echoes, signaling that the nightmare may be far from over.
