In the Cartoon Dimension, a mad scientist named Doctor Carnage conducts a shocking experiment on a man, disemboweling him and tearing his skull out through the stomach. Back on Earth, two sisters, Cindy and Candy, are left alone with Cindy’s friend Amy. As Cindy and Amy reach out to Rick and Eddie, Candy sits with a Terror Toons DVD she received in the mail. Created by Satan, the DVD promises wild adventures featuring Doctor Carnage and his partner Max Assassin, along with a lab monkey that has been mutated into a hulking cartoon gorilla.

Candy [Lizzy Borden] watches as the trio of friends tries to unwind with a playful Strip Ouija session, only for Carnage and Max to burst into her room. The horror quickly escalates: Candy’s spine is ripped out, Cindy’s friend Tommy is beheaded when he drops by, and a pizza delivery man is dismembered with a giant pizza cutter. The scene also includes a hypnotic disco number that makes Eddie vomit his own internal organs, a moment that underscores how far these cartoons will go. When the group attempts to flee, every door has been replaced by dizzying spirals, trapping them inside a nightmarish labyrinth. Rick is abducted and subjected to brutal brain experiments by Carnage and Max, while a released police officer from Terror Toons is blown up by a hidden dynamite in donuts, severing any chance of a quick escape.

The terror intensifies as Cindy is captured and forced to witness a gruesome act: Carnage and Max saw Amy in half as part of a grotesque magic routine. They then haul Cindy to a cartoonish Hell, where the Devil explains his plan to weaponize Terror Toons and corrupt the children of Earth. Facing the abyss, Cindy discovers that cartoons—where almost anything is possible—offer her a chance to become a superhero. The Devil sends her back to her house, and when Max reappears to attack, Cindy uses newly formed powers to snap his neck and crush his brain, sending a torrent of tiny monsters spilling from his split skull. Carnage charges with a giant axe, but she turns the weapon against him, severing his head and transforming the shattered remains into a swarm of microscopic threats that she then smashes with a crowbar against the Terror Toons DVD press.

As the carnage settles, Cindy and Candy’s parents return home from a wedding to a house in disarray: Rick is banging his head against a wall, and Cindy’s sister is nowhere to be found, all while Cindy laughs maniacally at the sight of the wreckage that used to be her family’s evening. Across the street, a neighbor’s child discovers another Terror Toons DVD in the mailbox and rushes inside, just as Carnage’s giggle echoes from the abandoned home. The final image lingers on the looming threat of more cartoons, leaving a chilling note that the nightmare could begin again.
