When Bill and Steve do arrive to carry out the hit, Paul survives the encounter and is rushed to the hospital, forcing Susan to press forward with her schemes rather than abandon them. Undeterred by the setback, she throws herself into a second, more elaborate plan. A biker named Bob is recruited to finish the job, while Sam’s partner in crime relies on a troubling twist: he enlists a former prostitute, Betty Johnson, to manipulate the hospital’s doctor into moving Paul into a private room where Bob can complete the killing in relative privacy. The manipulation hinges on seduction and deception, turning the hospital environment into a perilous battlefield where loyalties blur and danger lurks behind every closed door.

From the first signs of trouble, the plot is tested by a suspicious detective. Detective Scott, Bill Duke, begins to sense that Susan is connected to the failed murder attempt, and the tension rises as Penny Myers, Sam’s ex-wife, Lisa Edelstein, learns of the scheme and wants a larger cut of the life insurance money. As the plans unravel, both Bill and Steve insist on being paid for their “work,” even as the hits fail to go as planned. The film layers in fantasy sequences—visions of fear and paranoia from various characters—that offer comic relief while highlighting the characters’ anxieties about being caught, shot, or double-crossed. These dreamlike interludes illuminate each character’s inner fears as the plot threads threaten to come apart.

Despite the chaos, Bob manages to slip into Paul’s hospital room and smother him with a pillow, making the murder look finished and quick. He escapes into the night, but the following day brings a reckoning. Susan and Sam prepare to accompany their statements to the police and to settle their debts with Bill and Steve, only to find that security cameras and a growing web of evidence have sealed their fates. Detective Scott and a cadre of officers close in, arresting Susan, Sam, Bill, and Penny as footage from the hospital reveals the sequence of events. Bob tries to flee when he arrives at Susan’s home, opening fire on pursuing officers; he is shot and killed in the crossfire, and Steve dies from a stray bullet as the plan collapses. The many pieces of the scheme finally fall into place, and Susan and Sam are taken away to prison along with the others, while Betty manages to escape, leaving the others to face the consequences of their actions.

In the wake of the arrests, the bodies of Bob, Steve, and Paul are laid to rest at the local morgue. The story’s final image shifts to Betty, now living a freer life in Las Vegas as a casino cocktail waitress, seemingly out of reach of the law—for the moment. A chance encounter occurs when Dr. Stillman, Thomas Haden Church, happens to spot Betty at the casino with his wife while on vacation. He recognizes her but, wary of the repercussions a confrontation could have on his own life, chooses to pretend not to know who she is and continues his vacation, leaving Betty free for the time being. The film closes on a morally complex note, showing how cunning and self-interest can shape fates, even when the law catches up with the conspirators, and how one small slip can ripple through many lives.
