Desmond Niles abducts Ariel Wolfe and her friend Paul and forces them to locate a demon figurine called the idol of Baphomet inside the abandoned Vannacutt Psychiatric Institute. The assignment places them directly in a trap-rich environment where escape won't be easy. The stakes are survival and uncovering the idol's power.

Ariel, Paul, and Desmond's crew split up to search for the idol, but inmate ghosts start to interfere, killing Desmond's henchmen. The apparitions offer haunting glimpses of the inmates' suffering under Dr. Vannacutt. The supernatural presence shows the place is more than just abandoned ruins.
