Months after the previous film, Shinichi stays on the trail of parasites and uses Migi’s abilities to track and kill them. He maintains his uneasy balance between being human and being a host to a parasite. The ongoing threat to innocent people remains, driving him to protect those he loves.

Shiro, a reporter, begins following Shinichi as a possible target. He is hired by Ryoko Tamiya to uncover the truth about Shinichi and the son she birthed. The pursuit sets the stage for later revelations about Migi.

Shiro discovers the existence of Migi, the parasite living in Shinichi’s hand, and realizes the danger of exposing him. Shinichi warns Shiro to stay away for his safety and the safety of those around him. Tensions rise as the public eye turns toward their unusual alliance.

Ryoko Tamiya has given birth to a human son and begins learning what it means to be a mother. She grows more protective of her child and their chances of survival in a world of parasites. This personal transformation influences how she interacts with Shinichi and his allies.

Kuramori records a conversation with mayor Takeshi Hirokawa, who secretly leads a parasite-allied organization. Hirokawa orders the parasites to kill Kuramori’s daughter, though only she dies, revealing the brutal nature of the plan. The seeds of a vendetta are planted.

Kuramori kidnaps Tamiya’s human son and lures Tamiya to a park so she can witness the killing of the baby. Tamiya and Shinichi try to intervene, but Kuramori’s resolve remains firm. The moment marks a turning point in their struggle against the parasite network.

Despite her pleas to stop him, Kuramori is killed by Tamiya as she confronts him in the park. The act demonstrates Tamiya’s protective desperation and willingness to act against parasites. Shinichi is left to grapple with the consequences of vengeance.

In the ensuing chaos, the police fatally shoot Tamiya as she approaches Shinichi. She dies protecting her child and hands her baby over to Shinichi before dying. The loss underscores the cost of the battle between humans and parasites.

A secret service team evacuates the town hall after exposing Hirokawa as a human ally of the parasites. They kill nearly all of the parasites and Hirokawa himself, exposing the broader conspiracy. The immediate threat begins to recede, but not without leaving scars.

Goto, the remaining parasite host with five parasites inside, massacres the defense team and pursues Shinichi. Migi sacrifices himself to give Shinichi a chance to escape. The rescue attempt marks a turning point in Shinichi’s resolve to protect those he loves.

Shinichi mourns the loss of Migi, and Satomi Murano offers comfort and support during his grieving. The emotional strain deepens as he realizes his ties to humanity are more fragile than ever. Their bond provides a glimmer of hope amid the chaos.

The next morning, Shinichi discovers a remnant of Migi’s cells in his hand and uses that knowledge to track Goto to a disposal facility. He lures the parasitic monster there and fatally injures him, ensuring the safety of his loved ones. Migi also reveals he will enter hibernation to evolve, leaving Shinichi to face the world alone.

Years later, Shinichi and Satomi visit Taiki, the now-toddler son of Ryoko Tamiya. The encounter reflects a fragile peace that has formed in the aftermath of the battle. It hints at the enduring consequences of a world where humans and parasites coexist.

Uragami kidnaps Satomi and takes her to a roof, challenging Shinichi’s belief that humans can be saved. Satomi’s encouragement spurs Shinichi to resist, and with Migi’s brief help he saves her. The confrontation underscores the ongoing tension between humans and parasites.
