In rural Pennsylvania, a young girl screams for help after her leg is caught in an animal trap. A group of children arrives with an older woman they call Mama. Abraham knocks the girl unconscious, setting in motion a chilling ritual that will unfold later.

The children surround the girl and chant in a ritualistic manner under Mama's direction. Cynthia, obeying the twisted demands of the group, stabs the girl to death with a dagger. The murder marks the explicit first act of their satanic cult.

Years later, Nancy Johnson runs away after her stepfather Bert, a police officer, attempts to rape her. She leaves behind a threatening home life and looks to find safety elsewhere. The escape sets her on a perilous path.

Nancy is picked up by Hank and Tom, who are heading toward Nancy's sister's home in San Francisco. They travel through backcountry roads, facing hostility and the constant threat of danger. The ride becomes a tense, survival-focused journey.

In the backwoods, Hank and Tom pick up a Baptist preacher and his adult daughter Sandra, dropping them at a cemetery to visit the preacher's wife. A masked man with a machete murders the preacher while Sandra watches from a distance. The violence foreshadows a broader nightmare.

Later, Sandra returns to her home to find her father's corpse on the doorstep. The same machete-wielding killer attacks and murders her. The tragedy expands the danger beyond the immediate group led by Nancy.

At a local bar, Nancy, Hank, and Tom confront racist locals who refuse to serve Hank because of his race. With money running low, they steal groceries and are chased by two police officers. They flee by driving onto a dirt road and decide to camp in the woods.

In the morning, Nancy returns to the campsite to witness Luke and Abraham arresting Hank and Tom for murder. The officers shoot Hank and Tom to death execution-style and then pursue Nancy through the woods. She escapes deeper into the forest, terrified.

Nancy discovers a farmhouse where a teenage girl named Cynthia is playing cards while Cyrus dismembers two corpses in another room. Luke and Abraham return, lock Nancy in an animal cage next to Gwen, and Gwen recounts how they killed officers and stole their uniforms.

Luke, Abraham, and Cyrus return to the campsite and burn Hank and Tom's bodies. The grisly display of violence underscores the reach of the cult and the danger facing Nancy. The family unit at the farmhouse grows more ominous.

Luke has a chilling conversation with his dead mother, a decomposed corpse kept in a bed upstairs. The scene reveals a family haunted and guided by their own perverse rituals. It deepens the sense of isolation and horror surrounding them.

At midnight, the family conducts a Black Mass in the farmhouse and sacrifices Sharon, whom they have kidnapped. Nancy prays to God as Sharon is slain and her blood is offered to their dead mother in an attempt to resurrect her. The ritual marks a turning point for Nancy's fate.

The next morning, Luke and Abraham move Sharon's body to a field and begin a shallow grave. Bert stumbles upon the scene and overhears plans to sacrifice Nancy and Gwen on Easter. The danger intensifies as the cult's timetable becomes explicit.

During another Black Mass, Cynthia sacrifices Gwen while Nancy recites the Lord's Prayer. Luke drinks Gwen's blood, signaling a peak in the cult's power. Bert interrupts, confronting Luke, Abraham, and Cyrus, and orders Abraham to retrieve Nancy.

Cynthia attacks Bert, stabbing him to death as he intervenes. Bert's gun discharges, killing Cyrus and Abraham. Nancy escapes into a barn, overpowers Cynthia in a brutal struggle, and slashes her throat. She then pours gasoline on Luke and sets him on fire, ending his reign of terror as the ordeal concludes.
