Former CIA SAD/SOG officer John W. Creasy (played by Denzel Washington) arrives in Mexico, hoping to reconnect with his old friend Paul Rayburn, who manages a security firm. Paul persuades John to take a position as a bodyguard for a wealthy automaker, Samuel Ramos (portrayed by Marc Anthony). Samuel's young daughter, Lupita "Pita" Ramos (brought to life by Dakota Fanning), is in need of protection due to a concerning rise in kidnapping incidents — an alarming situation with one kidnapping occurring every hour in Latin America, and tragically, 70% of victims don’t survive.

Despite Pita’s mother, Lisa Ramos (Radha Mitchell), having turned down five prior candidates, she reluctantly chooses John, who is both an American and appears to be available at a lower cost due to his struggle with alcoholism. Lisa, sensing the pain buried within John, hopes his presence will provide protection for her daughter. Initially, John stays distant, avoiding connection with Pita as he battles his own demons and guilt regarding his past. Even during a dark moment when he considers ending his life, fate intervenes as the gun misfires. Gradually, he begins to bond with Pita, especially as he discovers her knack for swimming. After coaching her to overcome her fear of the starter gun, she triumphantly wins a swim meet.

However, tragedy strikes when John spots suspicious men in a car while waiting for Pita after her piano lesson. In a desperate attempt to protect her, he shouts for her to run, only to watch in horror as she freezes. The men attempt to abduct Pita, and even though John displays heroic courage, taking down three attackers and wounding a fourth at great personal risk, he is critically injured, and Pita is ultimately taken away.

As John recuperates, he faces wrongful arrest for killing police officers amid the kidnapping chaos. With help from AFI agent Miguel Manzano (Giancarlo Giannini), John is smuggled to a veterinarian clinic to shield him from corrupt officials. Recovering consciousness, John remains tight-lipped during questioning but eventually crosses paths with reporter Mariana Garcia Guerrero (Rachel Ticotin), who offers her investigative skills to expose the nefarious connections between the kidnappers and local law enforcement.

A ransom drop, orchestrated by the Federal Police, rapidly deteriorates into chaos when rival criminals ambush the kidnappers, resulting in severe violence. The Voice, the nefarious leader of the kidnappers (Roberto Sosa), becomes enraged after losing family members in the fray and threatens retaliation against Lisa and Samuel, declaring Pita will be forever lost to them as payback.
