Pambinthuruthu is an isolated island community where an illegal liquor operation runs under the radar of the law, led by a ruthless figure, V.R. Nadesan. The brew is tainted and dangerous, shipped off the island to cause injuries and deaths elsewhere. When a catastrophic tragedy claims 77 lives in a nearby city, the honest S.I. Balachandran tries to bring Nadesan to justice, but betrayal and violence swiftly derail the mission: he is mauled to death by Nadesan’s hounds, leaving the path open for years of unchecked power.

Twenty-five years pass, and Nadesan’s empire has grown wealthy and sprawling. On the surface, his operations are masked by legitimate fronts, including a glass bottle factory and a string of charitable fronts like the Nadesan Charity Hospital. He also runs a bar in front of a school, where local youths and staff frequently face harassment, all while the community watches.

Madhavan Nair [Nedumudi Venu] and Krishnan Nair take on the case, traveling to Pambinthuruthu to uncover the truth. They confront fierce resistance from Nadesan, who leverages the police chief—C.I. David—to arrest Krishnan Nair on a false charge of murdering a journalist who had been sniffing around the organ-trade scheme. In desperation, Madhavan Nair calls for help from his son, Madhura Raja [Mammootty], who arrives with resolve. He is joined by the adoptive brother Chinna [Jai Sampath], whose initial bravado lands him in trouble as he is arrested for attacking police officers.

Madhura Raja steps in, liberates Krishnan Nair, and brings in the reform-minded City Police Commissioner Rajendra Babu [Siddique] to pressure the corrupt apparatus and suspend C.I. David. With Chinna’s release secured, Raja swiftly moves to remove the two men from the field, sending his father and Krishnan Nair away while he stays back to confront Nadesan directly. The political landscape shifts as ego clashes intensify into a local assembly race, and Raja—an outsider with little public support against Nadesan’s philanthropic façades—plots to dismantle the illegal network that props up the villain’s popularity.

As the campaign unfolds, it becomes clear that Nadesan’s goodwill is built on a foundation of illicit activities, including covert organ trade conducted through the hospital’s free medical camps. The journalist who had been investigating the scheme is dead, a fate tied to the machinery that Nadesan has built to keep power secure. Raja leverages the truth, exposes the fake benevolence, and wins the election by a landslide, breaking the incumbent party’s long-running grip on power.

In retaliation, an enraged Nadesan orchestrates a brutal trap, murdering Chinna [Jai Sampath] and Lissy [Anna Reshma Rajan], the nurse who aided the journalist in gathering evidence. The same hounds that killed Balachandran return to exact vengeance, and Raja, heartbroken and furious, leads a relentless assault against Nadesan’s gang. He triumphs in a brutal confrontation, killing Nadesan in the same manner in which Balachandran, Lissy, and Chinna fell.

Three months later, Raja, now an elected MLA, sets his sights on higher ambitions, signaling a future where he hopes to become a minister—an explicit hint at a possible third installment in the saga.
