An elderly couple is enjoying a scenic drive through the picturesque Florida countryside, embracing the beauty of nature. Their journey takes a sudden turn when an old truck aggressively tailgates them, igniting their curiosity about the driver.

Curiosity drives the couple to follow the truck after it overtakes them. Upon encountering the driver near an abandoned church, they witness a shocking act—he is seen placing a covered body into a large protruding pipe, leaving them in horror at what they have just seen.

In a split second, the elderly couple's car nearly collides with the truck, causing them to veer off the road. This incident spurs them to turn back and investigate the eerie pipe that now haunts their thoughts.

In 2021, a millennial couple, Chase and Laine, embark on a trip to the Horror Hound festival in rural Louisiana. Unbeknownst to Laine, this trip will be a pivotal moment as Chase plans to propose, while she secretly suspects she may be pregnant.

As Chase indulges in his fascination with the local urban legend of the Creeper, the creature stirs from its slumber, hungering for new victims. This malevolent entity awakens every 23 years for 23 days, setting the stage for a terrifying encounter.

During a stop at a gift shop, Laine discovers an artifact that ignites unsettling premonitions, hinting at the dark events that lie ahead. The shop owner, Lady Manilla, cryptically comments on the couple's future, foreshadowing trouble.

At their hotel, Laine's attempts to confirm her pregnancy take an eerie turn when a crow crashes into their window. This unsettling moment heightens the already tense atmosphere as they prepare for the festival.

Upon arrival at the Horror Hound festival, Laine surprises everyone by expertly throwing a shuriken, showcasing a hidden talent. However, as the night unfolds, they realize that ominous events are unfolding around them.

Laine and Chase participate in a raffle hosted by Madam Carnage for a chance to stay overnight at a haunted plantation turned escape room. Unbeknownst to them, darker forces are at play as they unwittingly step into an ominous ritual.

As they venture towards the haunted house, the group crosses an old graveyard from the 18th century. Chaos erupts when the Creeper emerges, violently taking the cameraman and abducting Laine just as Chase prepares to propose.

Laine regains consciousness bound to a table, facing a horrifying fate as the Creeper prepares to pierce her abdomen with a knife. The tension mounts as the group faces off against the creature, sparking a desperate struggle for survival.

In a frantic bid to save themselves, the group struggles to confront the Creeper. Tensions peak when Stu shoots his weapon, accidentally drawing the creature's attention and escalating the chaos further.

Laine finally manages to escape and reunites with Chase and Stu, revealing her pregnancy and the Creeper's targeted interest in her. They uncover a shocking conspiracy: Madam Carnage and Lady Manilla have been worshipping the Creeper, luring victims into its grasp.

Realizing the danger, Laine devises a plan to lure the Creeper outside, giving Chase and Stu a chance to strike back. They use a fallen weathervane in a last-ditch effort to defeat the creature, culminating in a fierce battle.

The combination of Laine using the shuriken to blind the Creeper and the weathervane penetrating its body leads to a brutal conclusion. Yet, as they escape, the chilling roar of the Creeper echoes through the darkness, hinting at its ominous regeneration and future return.
