An archaeologist uncovers relics from an Indian burial ground, including the skull of an ancient chief. Skeleton Man Dominique Vandenberg materializes through a shimmering portal and immediately kills the archaeologist, then chases the archaeologist’s assistant to a nearby power plant, where she is slaughtered along with two workers. The threat is not just a superstition—it’s a relentless, almost spectral hunter, riding a horse and leaving a trail of violence in his wake.

Skeleton Man on horseback claims another life, killing a soldier and forcing his partner to retreat. Before the second soldier dies, he records a video and sends it back to his superiors, who respond by dispatching Delta Force to confront the unknown menace. The team moves in, but the confrontation is brutal and unyielding; a female soldier falls behind and is impaled, signaling that this foe operates with a relentless, almost mythic certainty.

A weary, seasoned frontline crew includes Captain Leary Michael Rooker, who must make sense of a threat that defies ordinary combat. An old Indian man, encountered during the mission, warns them that Cottonmouth Joe is a genocidal warrior who once slaughtered his own people and has returned to stalk the soldiers. The soldiers dismiss the warning at first, unsure how to square ancient vengeance with the modern weapons array they bring to the field.

Cottonmouth Joe continues his slaughter, hitting an oil pumping station and leaving the site in chaos. That night, two sentries vanish (marked as MIA), and the team’s scout, Staff Sgt. Oberron Casper Van Dien, also disappears into the night. The next day the squad finally comes face to face with the undead hunter. The heavy weapons specialist charges him in a bold move, only to be cut down. The squad returns fire, but their weapons seem to have little to no effect. A Citizen’s Militia helicopter supporting them is destroyed, and they later discover the heavy weapons hunter’s body is missing. The team also realizes the scout they had previously shot was captured by Cottonmouth Joe, who is using him as a grim target in a trap.

With dwindling ammunition, the squad attempts a series of traps that fail to contain the supernatural foe. One trooper is killed, and the team’s sharpshooter—Lt. Scott Sarah Ann Schultz—suffers a brutal death when her skull is crushed by a tomahawk. The remaining two soldiers, Captain Leary and Lt. Scott, press on, trying to bait Joe into a decisive mistake, but the hunter always seems to stay one step ahead.

The pursuit drives them to a chemical plant, where Cottonmouth Joe slaughters several workers, two guards, the plant manager, and multiple scientists. Law enforcement closes in, and Captain Leary wields a Sheriff’s Armsel Striker in the clash that follows. In a tense cat-and-mouse sequence through the plant’s corridors and rooms, Leary cornering the killer leads to a final showdown in a generator room, where he uses an electric current to annihilate the monster.

As the film’s credits begin to roll, a sudden reversal reveals that Skeleton Man may still be alive: he’s seen back on his horse in the woods, leaving the door open for a haunting implication that the threat persists beyond the frame.
