Mandy Pullman and Roy Seeley drift through a quiet Wisconsin lake at dusk, then spend the night camping on the shore. Their peaceful retreat is shattered when an unseen figure attacks them: Roy is killed and Mandy endures a brutal rape. The assault leaves Mandy severely injured, with a ruptured uterus and deep trauma, and she is rushed to the local hospital where Dr. Sam Cordell works to keep her alive. As the horror unfolds, the town’s uneasy mood deepens: in a separate thread, teenager Tim Galen is haunted by a recurring nightmare in which a woman is tormented by a monstrous figure, while his formidable grandmother Agatha Galen tries to steer him away from following the eerie dream’s pull.

At the hospital, Sam faces another layer of worry as the small community’s forces collide. Jenny Cordell, Sam’s teenage daughter, is dating Tim, a detail that adds personal tension to the investigation. Sheriff Hank Walden questions Sam about Mandy’s injuries, and a nosy local reporter, Laura Kincaid, shows up to push for answers, only to be told to back off. That night, the town’s library and museum becomes a crime scene when librarian Carolyn Davies is brutally raped and murdered just after closing. An autopsy reveals wounds eerily similar to Mandy’s, and the investigators uncover an unusually large amount of semen, a clue that ties the cases together in unsettling ways.

The investigation heats up as Sam and Laura press on. They discover that the semen from Davies does not match any known suspect, sending them toward a more puzzling explanation. Sam shares photographs of his deceased second wife and notes a startling resemblance to Mandy, deepening the sense that a shared history or a shared threat links the victims. As they dig deeper, they encounter accounts of a shapeshifting incubus—an entity said to enter the world through dreams and to take human form to prey on the living. Agatha Galen adds a grandmother’s cautionary voice, suggesting a long history of witch-hunter bloodlines in the Galen family and hinting that Tim’s dreams might be more than mere nightmares.

When Ernie Barnes and his two daughters are murdered at their farmhouse the following day, the fear in the town intensifies. Tim, still haunted by visions, seeks distraction at a local cinema, only to witness another brutal crime: a young woman is raped and killed in the downstairs bathroom, the stall door bent beyond repair as a gruesome sign of the violence. Hank Walden and Sam arrive at the latest crime scene, while Laura offers a possible historical angle—Satanic records that echo through the town’s past—suggesting a long-running thread of ritual crimes.

Late that night, the group gathers again at a tense crossroads. Laura reads from an old volume detailing the incubus, a shapeshifter that can manifest in human form and manifest through dreams. Agatha reveals more about Tim’s mother, who died before his birth and was blamed for witchcraft due to her psychic powers, reinforcing the idea that the Galen family bears a legacy of hunting supernatural threats. The night deepens the mystery as Tim confronts the meaning behind his dreams and the town’s buried legends.

In the climactic finale, Sam attempts to coax Tim into another dream to prove the connection between visions and the murders. Tim succumbs to a trance and races toward Jenny’s room, wielding a dagger given by Agatha. Sam intervenes, but the struggle ends with Tim’s death. The confrontation takes a darker turn when Laura’s face briefly shifts into that of the incubus, revealing that the predator has been hiding in female form all along. As Laura embraces Sam, he looks over her shoulder and discovers Jenny’s lifeless body lying on the bed, a shocking reminder that the nightmare has become all too real.
