As the clock struck midnight on October 31, 1931, chaos erupted at the Vannacutt Psychiatric Institute for the Criminally Insane. Inmates revolted against their sadistic overseer, Dr. Richard B. Vannacutt, leading to a catastrophic prison riot that resulted in the deaths of every patient and nearly all staff members, marking a devastating day for the institution.

Fast forward to 1999, we are introduced to Evelyn Stockard-Price, who is stuck in a crumbling marriage with her husband, Steven Price. While Evelyn seeks to rekindle their relationship, Steven is preoccupied with elaborate pranks, adding further strain to their already fractured bond.

Amidst her distress, Evelyn persuades Steven to host her birthday party at the abandoned Vannacutt hospital. Watson Pritchett, the estate’s owner, warns Steven about the ominous past of the property, having once resided there during its brief period as a private home, foreshadowing the horrors to come.

As the party proceeds, five uninvited guests mysteriously appear: film producer Jennifer Jenzen, baseball player Eddie Baker, former TV star Melissa Marr, doctor Donald Blackburn, and Pritchett himself. The mysterious arrival creates tension as it becomes clear that neither Evelyn nor Steven invited these guests, and a tantalizing $1 million prize hangs in the air for any guest who survives the night.

The atmosphere takes a dark turn when an unseen force disrupts the building's security, locking all exits and trapping the guests inside. Tensions escalate as Steven, blaming Evelyn for the predicament, turns aggressive, creating an environment of fear and distrust among the attendees.
