On October 8, 2009, Hell House, a haunted attraction in Abaddon, Upstate New York, claimed the lives of 15 people in what authorities call a 'severe malfunction.' The incident left the community shaken and cast doubt on the venue's operation. A documentary crew arrives later, chasing answers and trying to understand what went wrong.

A documentary producer, Diane Graves, tracks down Sara Havel, the sole surviving Hell House staff member. Sara agrees to share uncut behind-the-scenes footage. Her testimony promises to reveal what happened in the run-up to the tragedy.

Sara's tapes chart the team's arrival at the dilapidated Hotel Abaddon in Rockland County, New York as they prepare for Halloween. The plan centers on a basement finale with a chained actress, intended to heighten the scare. The crew also hires local scare actors to bring the show to life.

The Hotel Abaddon was built by cultist Andrew Tully, who chose the site for its demon-gateway symbolism. In 1989, a mother and her 11-year-old daughter disappeared from the hotel, and although Tully was cleared, the hotel's reputation spiraled. He hanged himself soon after, leaving a legacy of whispers and dread.

As Hell House goes up, the crew begins capturing undeniable supernatural phenomena. They report an autonomous clown mannequin, flickering shadowy figures during strobe tests, and moments where crew members appear briefly possessed. The line between filming and fear grows as the pressure to open the show increases.

Paul contemplates quitting the project due to mounting unease and fear. His loyalty to director Alex keeps him in place for the shoot. The crew’s morale frays as the haunting intensifies.

During a late-night encounter with a ghost girl, Paul vanishes without a trace. The crew later discovers him in the basement, withdrawn and unresponsive, adding to the escalating dread.

Tensions rise as Tony quits after Alex’s relentless push to finish the project. Mack convinces him to stay long enough to complete the filming, but the strain proves too much. The team edges toward the looming opening night.

The attraction opens to a large crowd. In the basement, Melissa is brutally killed by a shadowy ghost cultist, triggering a chaotic stampede among guests. Tony is slain while trying to protect Melissa, and Mack and Sara flee toward the attic.

Mack and Sara manage to escape to the attic, where they discover Alex hanged as the chaos unfolds. The scene grows more tangled as shadow cultists close in, and the footage from the night ends abruptly.

In the present, Sara asks for a break from the interview, hinting she will await the crew in her hotel room labeled 2C. She suggests they enter the Abaddon Hotel to witness the crime scene firsthand. The investigation shifts from the past to a confrontation with the haunting history she captured.

Diane and her Cameraman head to the hotel, while Mitchell catalogs Sara’s footage back at the studio. He uncovers footage showing that after Mack’s death, a possessed Paul slaughters Sara and then cuts his own throat.

Diane and her companion break into the hotel, ignoring calls to retreat, and confront the aftermath on the second floor. They discover a room labeled '2C' and find Sara sitting with her back to them, before being attacked by a ghoulish Sara and two shadow cultists, ending the documentary on a chilling note.
