Three years after the Hillcrest murders, Laurie Strode is institutionalized at Grace Andersen Sanitarium. She is guilt-ridden and anticipates Michael's return, setting a trap on the sanitarium roof. This setup foreshadows a confrontation that could decide her fate.

Michael appears on the sanitarium roof and triggers Laurie's trap. The device momentarily incapacitates him, giving Laurie a brief sense of control. However, she hesitates to remove his mask to confirm his identity, sealing her fate.

Laurie attempts to verify Michael's identity, but he stabs her and throws her from the roof to her death. He then hands his knife to a mute mental patient and frames him for the murders of Laurie and two security guards. The return of Michael Myers is solidified by Laurie's demise.

A year later, Sara Moyer, Bill Woodlake, Donna Chang, Jen Danzig, Jim Morgan and Rudy Grimes win a competition to appear on the Dangertainment reality show. They travel to Michael Myers' abandoned childhood house in Haddonfield to uncover what led him to kill. Cameraman Charlie is killed by Michael, signaling the killer's return.

On Halloween night, the contestants split into groups to search for clues inside the house. Michael suddenly appears and kills Bill, proving the danger is real and immediate. The remaining players realize they are not safe.

Donna and Jim realize the show's premise is a setup when they discover a wall filled with fake corpses. Michael arrives and kills Donna before they can warn anyone else. The reality of danger hits home for the others.

Jen discovers Bill's corpse and is decapitated by Michael in front of Rudy, Sara, and Jim. The trio understands that the killer is real, not a staged prank. Fear and panic spread as they scramble to respond.

Freddie enters the house dressed as Michael to scare the competitors. The real Michael mistakes him for Charlie and shoves him toward Nora in the garage. The confusion blurs lines between showmanship and murder.

After spotting Freddie in the Michael disguise, he explains the actual scheme and begs the contestants to cooperate for a payoff. Rudy, Sara, and Jim decide to gather the others and leave the house to escape. The group faces a grim choice between safety and the show's supposed payoff.

Michael moves through the house and kills Jim and Rudy in rapid succession, forcing Sara to hide and run. He chases her upstairs, turning the escape attempt into a deadly game of cat and mouse. The survivors realize that escape may be nearly impossible.

Deckard begins sending Michael's locations to Sara to help her avoid him, turning the pursuit into a coordinated effort. Sara uses the information to slip through safe paths and stay one step ahead of the killer. The plan buys her crucial moments to survive.

Sara runs into the tunnels and discovers an exit leading to the garage, where she finds Nora's body. Michael resurges and attacks again, threatening to close the escape route for good. The tension peaks as the cycle of pursuit continues.

Freddie returns and fights Michael while an electrical fire breaks out in the garage. He manages to electrocuted Michael long enough to carry Sara to safety, leaving Michael to perish in the burning space. The immediate danger appears to be over, but the fate of Michael remains uncertain.

Back at the morgue, Michael is believed dead as the coroner prepares to examine the body, but he suddenly awakens. In an alternate ending, Sara demands to see the corpse, opens the body bag, and Michael wakes again; Sara then kills him with an axe. Both endings leave Michael's fate unresolved and open to interpretation.
