In 1987, Dr. Ben Carson, Cuba Gooding Jr, travels to Ulm, Germany to meet Peter and Augusta Rausch, the parents of twins conjoined at the back of their heads. He believes a separation is possible and could save both lives, but the procedure carries immense risk: one or both babies could die. After laying out the stakes in clear terms, Cuba Gooding Jr agrees to operate, determined to give the twins their best chance at a normal future. The narrative then shifts back to 1961 in Detroit, Michigan, where we meet an 11-year-old Ben Carson. He is struggling in school, and his mother Sonya, a woman with a deep belief in education, becomes the quiet force behind his transformation. Sonya, Kimberly Elise Carson, is distressed by her sons’ academic struggles and decides to intervene in a very hands-on way. She imposes a strict routine: Ben and his older brother Curtis must master the multiplication tables, cut back television time to two shows a week, and commit to reading books and writing reports on them. What they don’t know is that Sonya herself hides a painful secret—she is illiterate—and this binds her even more tightly to the siblings’ education. The consequences are profound: under her guidance, Ben and Curtis discover the world of books and ideas, and in just one year, Ben rises from the bottom of his class to the top.

At the 8th-grade awards ceremony, a pivotal moment underscores the unequal social pressures surrounding Ben. There, he meets his future wife, Candy Rustin, whose support becomes a steady source of strength as he tackles demanding studies in neurosurgery. Aunjanue Ellis-Taylor plays Candy, a collaborator who stands by him as he navigates an elite, demanding environment. After Yale, Ben earns a place as a resident at Johns Hopkins Hospital, where he faces a career-defining dilemma: operate on a dying patient without explicit permission or supervision, risking professional consequences in the process. He chooses to take the risk, saving the man’s life and earning the respect of his supervisors, which accelerates his rise within the medical world.

In 1985, a personal tragedy deepens the story. Ben’s mother joins the family in Maryland, and Candy experiences a miscarriage of twins. The emotional strain is tempered by Ben’s professional triumphs, including a breakthrough moment when he operates on a four-year-old girl who convulses hundreds of times a day. The procedure, a hemispherectomy, removes half the brain in order to stop the seizures. The operation is a dramatic success, and it brings with it a new wave of media attention that foreshadows the public spotlight to come.

Returning to 1987 in Ulm, Germany, Ben approaches the long-awaited twin-separation operation with renewed focus. For months he has grappled with a plan, and an epiphany arrives as he plays billiards alone: a new approach crystallizes, offering a viable path to safely separate the twins. The operation stretches over a long night; about 22 hours into the procedure, Ben and his surgical team execute the plan and succeed in separating the twins, saving both lives and delivering relief to their grateful parents, Peter and Augusta. The film closes with Ben Carson surrounded by the press and reflecting on the journey—from a difficult Detroit childhood to a career built on precision, risk, and hope.
