Four teenagers—Ginny, Phil, Sandra, and Tim—discuss the notorious serial killer, The Blissfield Butcher, who has been active since the 1970s. The Butcher brutally murders Tim, Sandra, Phil, and Ginny in Ginny's house, leaving the mystical dagger La Dola missing. This marks the beginning of his reign of terror as the police remain unable to catch him.

The following day, Millie Kessler wakes up for school, living with her mother Paula and sister Charlene. It has been a year since their father's death, affecting their relationships. Millie faces low self-esteem and navigates her high school environment, including her best friends Josh and Nyla.

At school, Millie faces humiliation from her teacher Mr. Bernardi and antagonism from the popular girl Ryler. During a football game, Millie is attacked by the Butcher and is chased onto the field, where he stabs her with La Dola. Charlene intervenes by shooting at the Butcher, causing him to flee, but Millie is left injured and disturbed.

On Friday the 13th, the Butcher wakes up in Millie's body, displaying menacing behavior. Simultaneously, Millie finds herself in the Butcher's body near the old mill, horrifying her as she realizes the body switch has occurred. The switch is alarming due to the public's awareness and her limited time to reverse it.

Disguised as the Butcher, Millie attempts to hide at school while the real Butcher, in Millie's body, tries to kill Ryler but ends up freezing her to death. Millie reveals her identity through a cheerleading routine, convincing her friends she is still herself. Time becomes crucial as they learn only nine hours remain to switch back.

Millie, as the Butcher, confronts her teacher Mr. Bernardi and fights him to prevent his killing. Chaos erupts as the Butcher's presence leads to police intervention at the school, and Millie manages to escape with her friends. Meanwhile, the switch becomes increasingly dangerous as the 24-hour window nears its end.

Millie, with her friends' help, arranges to retrieve La Dola from the police evidence locker to reverse the body switch. They head to the old mill where the dance is happening, knowing that with only minutes left before the switch becomes permanent, they must act quickly to save her and stop the Butcher.

At the old mill, chaos ensues as the Butcher, in Millie's body, attacks multiple people including Brett and Booker. Millie and her friends attempt to subdue him, with Booker setting an alarm to track the time. The police arrive, and a desperate battle takes place as they try to prevent the body switch from becoming permanent.

Realizing they are nearly out of time, Millie uses La Dola to transfer the Butcher back into his original body, effectively ending his reign of terror. Police shoot the Butcher, and Millie is relieved to regain her own body. She and Booker share a heartfelt kiss, marking a hopeful end to their ordeal.

Back home, Millie is recovering with her family. However, in a shocking turn, the Butcher, still alive, attacks Millie, Charlene, and Paula. Despite their efforts to fight him off, he overpowers them until Millie manages to impale him with a broken table leg, killing him once and for all, asserting her strength and resilience.

With the threat eliminated, Millie reflects on her strength and identity, declaring her resilience with a defiant remark. The film concludes with her celebrating her recovery and newfound confidence, as she and Booker enjoy a normal moment, free from the terror of the Butcher.
