In a dystopian future, society enforces a strict one-child policy imposed by a powerful corporation known as MenTel. Married couples are only allowed to conceive once, and any subsequent pregnancies are not permitted, even if the first child does not survive. This oppressive regime ensnares couple John and Karen Brennick, portrayed by Christopher Lambert and an uncredited actress as Karen. After one failed pregnancy, they learn they cannot try for another; however, Karen discovers she is pregnant again.
The film opens with John and Karen attempting to cross a security checkpoint as they journey to Canada. They cleverly attempt to safeguard Karen’s abdomen from detection, but the guards grow suspicious and try to halt their escape. In a desperate bid for freedom, Karen makes a run for the border, but John is captured and sent to the MenTel prison. While on a transport to the penitentiary, another prisoner questions John’s bizarre smile despite receiving a 31-year sentence. John responds, “I’m happy because Karen escaped.”
Upon arrival at the penitentiary, a vast and treacherous underground facility, John is thrust into a brutal environment filled with harsh treatments and severe punishments. He undergoes a nightmarish check-in process that includes the insertion of an “intestinator”, a device that inflicts excruciating pain when punishment orders are enforced. This device can even lead to fatal consequences, a grim reality John witnesses firsthand when a fellow inmate’s abdomen explodes after crossing a designated safe zone.
As John adjusts to his new life behind bars, he finds himself in a cell shared with dangerous inmates, including the menacing Stiggs. Forced to endure the indignity of sleeping on the floor, John is haunted by dreams of his wife, only to be interrupted by a prison control system known as ZED-10, which intrudes into the inmates’ thoughts. The prison is overseen by a tyrant named Poe, who manipulates ZED’s operations.
John is soon thrust into forced labor, partaking in the prison’s construction efforts, where terrifying confrontations arise. He faces off against Maddox, an imposing inmate marked with the tattoo “187,” symbolizing murder. During a brutal fight, John must grapple with his own survival while ZED issues punishment in a shocking display of power, leading to the death of another inmate and John’s own further punishment.
As the harrowing days pass, John finds himself subjected to horrifying torture in isolation, strapped onto a device that spins him relentlessly, filling his head with hallucinations. In his darkest moments, he is visited by Karen, who had learned to infiltrate ZED’s control through manipulation of Poe. With her intervention, John eventually breaks free from his catatonic state and rises to a position of leadership among the inmates.
Meanwhile, a plan for their escape begins to take shape, led by John and aided by allies including Abraham, a fellow inmate who works for Poe. Together they navigate perilous grounds, utilizing stolen technology to outwit their captors. In a turn of fate, Poe becomes infatuated with Karen, leading to further complications as they unearth ominous revelations regarding the fate of children born to imprisoned mothers.
As tensions escalate, John and his crew execute their escape plan amid chaos, but tragedy strikes when they encounter the cybernetic soldiers created from former inmates. Despite significant losses, including the death of Stiggs, John and his remaining allies forge on, fighting against their mechanized foes while confronting the horrors instigated by Poe and ZED.
A climactic confrontation with Poe reveals the depths of his villainy, ultimately culminating in a desperate rescue attempt where John fights to save both Karen and their unborn child. With a nail-biting rush to the surface, they evade the relentless pursuit of ZED’s machinations, facing life and death in a crumbling facility.
In a desperate twist of fate as they attain freedom, Karen goes into labor. Tragedy looms as the stolen MenTel truck, still under ZED’s control, threatens their lives. In a tense moment, John defiantly fights back, leading to a dramatic explosion that pits love against sacrifice. Ultimately, against all odds, John locates Karen and their newborn. Together, they gaze into the eyes of their child, filled with hope for the future they fought so hard to attain.
