After narrowly escaping a perilous mission in Dhaka, Tyler Rake retreats to a tranquil cabin in Austria to recuperate from his ordeal. However, his peace is short-lived when a stranger approaches him with a dire request: he must rescue Ketevan, the sister of his ex-wife Mia, along with her two children, Sandro and Nina. Ketevan’s husband, Davit Radiani, is a notorious co-leader of the Nagazi, the largest crime syndicate in Georgia, a group he founded with his brother Zurab. Following the murder of a DEA agent, Davit finds himself imprisoned, unable to escape even with his criminal connections strained by the increasing pressure from U.S. authorities. Desperate, Davit coerces Ketevan and their children to live in the oppressive prison conditions, where they endure his abusive reign. In a moment of desperation, Ketevan reaches out to Mia for assistance.

Determined to save them, Tyler assembles a team, enlisting the help of Nik and her brother Yaz. Together, they devise a daring plan to infiltrate the prison by bribing a guard. But their operation quickly spirals out of control when the prisoners discover their presence, igniting a chaotic riot. Amid the turmoil, Davit confronts Tyler and Ketevan, leading to a fateful encounter where Tyler kills Davit in self-defense. As the chaos unfolds, Tyler and Ketevan navigate through the riot and manage to escape on an armored train. Their journey takes them to Vienna aboard a plane, though it’s soon revealed that Sandro, who had idolized his father and was heavily influenced by the Nagazi’s cult-like ideology, secretly contacts Zurab to disclose their whereabouts. This betrayal leads Zurab and his men to mount a dangerous attack on Tyler and his team at their safehouse in DC Towers, culminating in a furious confrontation. Ketevan berates Sandro for his reckless choice that put them all in jeopardy, and in the confusion, he ultimately abandons her, opting to join his father’s brother Zurab.

In a harrowing chase, the team escapes in one of Zurab’s helicopters, but not before Yaz sustains critical injuries at the hands of Zurab’s men. They retreat to Tyler’s cabin, where the reunion with Mia offers a bittersweet moment. Tyler expresses heartfelt remorse for having left for a mission before their son succumbed to cancer, a final memory that haunts him. Meanwhile, Zurab’s right-hand man, Avtandil, urges restraint, advising Zurab to reconsider his quest for vengeance to avoid further losses and to set a positive example for young Sandro. But Zurab, unwilling to accept his setbacks, murders Avtandil and confronts Tyler, challenging him to meet at a nearby airfield adjacent to a church. Tyler heads to the airfield while leaving Nik behind to shield her from danger.

The climactic showdown approaches as Tyler dispatches the remaining members of the Nagazi and discovers Zurab and Sandro in the church, with Sandro strapped to an explosive vest. In a tense standoff, Zurab tries to compel Sandro to shoot Tyler, but the boy, realizing the true nature of his father and uncle, finds the courage to resist. Just in time, Nik arrives to defuse the vest, allowing Tyler to engage Zurab in a final confrontation. After a fierce battle, Tyler emerges victorious, killing Zurab. However, this victory is bittersweet as Tyler and Nik are subsequently apprehended by the police and thrown into prison. In a poignant visit, Mia informs Tyler that Ketevan and her children have been placed in protective custody, although their assets are frozen. Tyler instructs Mia to provide them with $1 million in cash concealed beneath the floorboards of his cabin. Mia reassures Tyler that their son passed away believing his father was a hero. As Tyler and Nik are brought out of prison, they are approached by the stranger once more, who offers them another assignment in exchange for their freedom.
